Usefulness of MR arthrography of the hip with leg traction in the evaluation of ligamentum teres injuries

Abstract: MR arthrography with leg traction offers accurate diagnosis of ligamentum teres injuries. Patients with complete tears of the ligamentum teres exhibit increased articular distraction that may indicate secondary hip instability.

“…MRI, while less sensitive has been shown to also have some utility in reaching a preoperative diagnosis of injury (20, 23). MRI arthrography with traction has been shown to have greater levels of sensitivity and specificity for both low grade and high-grade tears compared with MRI alone (24). Notably, in all of these studies arthroscopy has been used as the gold standard to determine the presence or absence of LT pathology.…”

“…MRA with axial traction has been evaluated with success in the shoulder, [11][12][13] hip, [14][15][16][17][18][19][20][21] knee, 22 wrist, [23][24][25][26][27][28][29][30] ankle 31,32 and metatarsophalangeal 33 and metacarpophalangeal joints. 34 To the best of our knowledge, there is no study comparing MRA of the elbow with and without traction.…”

“…Initial results of traction MR arthrography of the hip showed high accuracy in detecting chondrolabral lesions, lesions of the ligamentum teres, and intra-articular loose bodies. [132][133][134][135] In our experience, traction is not only helpful in detecting cartilage delamination but also in visualizing the whole extent of cartilage damage in older patients with mild radiographic joint degeneration. In these patients the referring surgeons request advanced imaging with traction to determine whether a joint-preserving surgery is still a reasonable treatment option based on the already present cartilage degeneration (►Fig.…”
