Using Directional Deep Brain Stimulation to Co-activate the Subthalamic Nucleus and Zona Incerta for Overlapping Essential Tremor/Parkinson's Disease Symptoms

Abstract: This index case report describes a novel programming approach that utilizes the 8-contact directional Deep Brain Stimulation (DBS) lead to effectively control the akinesia, rigidity and tremor of Parkinson's Disease (PD), as well as a severe kinetic tremor of Essential Tremor (ET), in a patient with overlapping symptoms of both PD and ET. Through utilizing a bipolar directional montage on a single segmented contact, symptom control was attained via likely co-activation of the Subthalamic Nucleus (STN) and the … Show more

“…30 Directional leads may provide better control of both dystonia and tremor by stimulating STN and the cerebellar pathway simultaneously with a minimal adverse effect. 31 Patients with DT and patients with ET share many clinical characteristics. Patients with ET frequently have alcohol responsiveness and positive family history.…”

“…We have received the commentary by Wu et al (1) of our case report (2) and welcome the discussion and alternative analysis. To summarize, we presented a patient with combined features of Parkinson's and Essential Tremor, both treated with a single unilateral subthalamic nucleus (STN) implant.…”
