Using Tc-99m Pertechnetate Scintigraphy to Predict the Outcome of Sialoendoscopy in Obstructive Submaxillaritis

Abstract: In patients with submaxillaritis, whether a sialoendoscopy should be performed depends on the results of salivary gland scintigraphy.

“…28 Researchers are currently investigating the factors that contribute to the success rate of sialendoscopy, with a recent prospective study reporting that it may be possible to predict the outcome of sialendoscopy using Tc-99m pertechnetate salivary gland scintigraphy. 25 With more research, endocrinologists and otolaryngologists may be able to further improve the success rate of sialendoscopy by identifying which patients will respond favorably. Nonetheless, even though this technology is relatively novel, success rates largely remain high.…”

“…With a heightened awareness of the incidence and minimally invasive treatment options available for radioactive iodine induced sialadenitis, physicians may be able to reduce the severity and/or incidence of sialadenitis following radioactive iodine therapy with referral to physicians performing this procedure. 23,25 Recurrence rates for sialadenitis following sialendoscopy ranged from 0% to 40%, as shown in Table 2. The variability in the recurrence rates may in part be due to the small sample sizes from each study.…”

“…(2009) used sialoendoscopy for the treatment of recalcitrant pain/swelling after 131 I therapy, and symptoms were improved in 75% of the patients with no serious complications. Xu et al . (2010) evaluated 99m Tc pertechnetate scintigraphy to determine which patients should have sialoendoscopy to relieve obstruction of the submandibular gland.…”
