Using Telehealth Technology to Serve Patients Reporting Sexual Assault

Abstract: Many communities across the country are developing, implementing, or already operating programs that provide patients with access to sexual assault nurse examiner (SANE) expertise and care through telehealth technology (e.g., teleSANE or teleSAFE). The speed at which teleSANE programs are proliferating is outpacing the available research and evaluation to inform key decisions on program development and implementation. “…The application facilitated the nurse's understanding, mitigating the professional's feeling of powerlessness and supporting their clinical thinking for the execution of the nursing process based on scientific evidence, corroborating the study by Alves et al (2021)34 which has developed a similar application. The feasibility of a telehealth care program was the focus of the study by Shaw et al (2022)25. The program, in addition to being viable, proved to be promising, since it guided the main decisions that must be made in the development and implementation of the program's actions.…”

“…Access to SANE led-exams is also limited as many communities across the United States, especially rural areas, have no or few SANEs available due to factors including lack of funding for specialized training and administrative support as well as strains on the nursing workforce (United States Government Accountability Office [GAO], 2016). To help combat the shortage of trained SANES, providing SA exams via telehealth with trained SANEs has been a model of care increasing across the country Miyamoto, Thiede, Wright et al, 2022;Shaw et al, 2022;Walsh et al, 2019).…”

“…Telehealth models of care show promise in ensuring individuals have equitable access to trained SA providers regardless of where they live Miyamoto, Thiede, Wright et al, 2022;Shaw et al, 2022;Walsh et al, 2019). Similarly, the telehealth model may also ensure that the SANE can provide adequate discharge planning including connection with local resources and follow-up care.…”

“…The evaluation focused on specific changes made to the typical teleSANE model for rapid implementation of the Temporary Model; the preparation for and roll‐out of the Temporary Model; the patient and provider experiences in implementing the Temporary Model; and what the evaluation findings mean for the future care of patients reporting sexual assault. Details on the specific changes made to the typical teleSANE model are presented in a separate manuscript (Shaw et al, 2022). This manuscript reports on the evaluation findings related to preparing for and implementing the Temporary Model; the patient and provider experiences; and implications for future medical forensic care of patients who present following a sexual assault.…”

“…; and (3) what should the future of SANE care look like? Evaluation findings on the specific features altered for the Temporary Model are reported in a separate manuscript(Shaw et al, 2022). From May 27, to July 13, 2020, the MPDH SANE Program and NTC provided teleSANE care in five hospital EDs that had previously only received an in‐person SANE response.…”
