Utility of First Dorsal Metacarpal Artery Flap for Thumb Defects

Abstract: Background Reconstruction of thumb tip, while providing a durable and sensate flap, is a challenging task. It is important as thumb accounts for 50% of hand functions. Options for coverage are: healing by secondary intention, local/regional flaps, microsurgical toe wraparound flap, etc. First dorsal metacarpal artery (FDMA) flap is one regional flap that has been used for thumb cover, usually for defects till interphalangeal joint or just distal to it. We present our case series for FDMA flap and its variation… Show more

“…[1][2][3][8][9][10][11][12] Flaps which can be based or elevated from the dorsum of the finger are extended reverse dorsal metacarpal artery flap, dorsal digital artery perforators flaps, kite flap, boomerang flap, intercommissural flaps, eponychial flap (which gives relative nail lengthening and better aesthetic appearance), and first dorsal metacarpal artery (mostly used for thumb defects). 9,[11][12][13][14][15][16][17][18][19] DMAP flap is based on the perforator which usually arises from the dorsal metacarpal artery at the level of the metacarpal neck distal to the juncture tendinae.…”

“… 4 The major disadvantage is venous insufficiency occurring in the flap. 1 4 Venous insufficiency and necrosis are inevitable in cases where the width of the pedicle is narrow and the pedicle is compressed ( Fig. 1 ).…”
