Utilization of p40 (ΔNp63) with p63 and Cytokeratin 5/6 Immunohistochemistry in Non-Small Cell Lung Carcinoma Fine-Needle Aspiration Biopsy

Abstract: Objective: Specific subclassification of pulmonary non-small cell carcinoma (NSCCA) is clinically necessary, and the aim of this study is to examine the utilization of p40 (ΔNp63) in fine-needle aspiration (FNA) biopsy for lung NSCCA. Study Design: Database files of the Washington University Medical Center were searched. Patients who underwent endobronchial ultrasound and CT FNA of a primary lung neoplasia were selected and immunohistochemistry (IHC) was performed. A panel of markers was utilized, including p4… Show more

“…Collins et al, studied p40 and p63 expression in FNA specimens in patients with primary pulmonary NSCLC, P40 sensitivity and specificity were: 89.4% and 100% while P63: 86.8% and 96.7%. [35]. P40 specificity and sensitivity for SqCC in FNA cell blocks were both 100% in FNA specimens in Vogt et al series, in the same study P63 sensitivity and specificity were 97% and 80% respectively.…”

“…A recent study has demonstrated the role of P40 as a marker for sebaceous lineage, noting that this antibody can be utilized for diagnosing sebaceous carcinoma in the setting of poorly differentiated carcinoma [30]. P40 is also expressed in the cuboidal tumor cells of sclerosing hemangioma of the lung, but not in the polygonal tumor cells [31,34,35]. …”

“…Although many types of nonpulmonary adenocarcinomas (such as endometrium, pancreas, breast and ovary) may be less than 50% positive for CK5/6 [43], it has been reported to be a sensitive and specific marker of lung squamous cell carcinomas (at 73-100% in tissue sections [11,44,45,46,47] and at 75-100% in cytology [41,48,49]). In our study, CK5/6 showed the best possible specificity (100%) for squamous cell carcinoma, but the sensitivity was 81%.…”

“…p40 expression has been reported to be specific for squamous cell carcinoma of the head and neck, esophagus, thymomas, urothelial carcinomas and lung squamous cell carcinoma [53,55,56,57,58]. Immunocytochemistry for p40 has been applied to cytology samples, such as cell blocks and fine-needle aspirates [59,60,61], with Collins et al [49] describing p40 as having 89% sensitivity and 100% specificity for non-small cell lung carcinoma. In our study, the sensitivity and specificity for p40 in squamous cell carcinoma were 84 and 98%, respectively (table 3).…”
