Background As the organisation of health and social care in England moves rapidly towards greater integration, the resulting systems and teams will require distinctive leadership. However, little is known about how the effective leadership of these teams and systems can be supported and improved. In particular, there is relatively little understanding of how effective leadership across integrated care teams and systems may be enacted, the contexts in which this might take place and the subsequent implications this has on integrated care. Objective This realist review developed and refined programme theories of leadership of integrated health and social care teams and systems, exploring what works, for whom and in what circumstances. Design The review utilised a realist synthesis approach, informed by the Realist And Meta-narrative Evidence Syntheses: Evolving Standards (RAMESES) publication standards, to explore existing literature on the leadership of integrated care teams and systems, complemented by ongoing stakeholder consultation. Empirical evidence specifically addressing leadership of integrated teams or services was limited, with only 36 papers included in the review. The evidence collected from these 36 papers was synthesised to identify and build a comprehensive description of the mechanisms of leadership of integrated teams and systems and their associated contexts and outcomes. Consultation with key stakeholders with a range of expertise throughout the process ensured that the review remained grounded in the reality of health and social care delivery and addressed practice and policy challenges. Results Evidence was identified for seven potentially important components of leadership in integrated care teams and systems. These were ‘inspiring intent to work together’, ‘creating the conditions to work together’, ‘balancing multiple perspectives’, ‘working with power’, ‘taking a wider view’, ‘a commitment to learning and development’ and ‘clarifying complexity’. No empirical evidence was found for an eighth mechanism, ‘fostering resilience’, although stakeholders felt that this was potentially an important, long-term component of leadership. A key message of the review was that empirical research often focused on the importance of who the leader of an integrated team or service was (i.e. their personality traits and characteristics) rather than what they did (i.e. the specific role that they played in integrated working), although stakeholders considered that a focus on leader personality was not sufficient. Other key messages highlighted the way in which power and influence are used by integrated service leaders and identified the hierarchies between health and social care which complicate the leading of integrated teams and systems. Limitations Evidence specifically addressing leadership of integrated care teams and systems was limited and lacking in detail, which restricted the degree to which definitive conclusions could be drawn around what works, for whom and in what circumstances. Conclusions Research into the leadership of integrated care teams and systems is limited and underdeveloped, with ideas often reverting to existing framings of leadership in which teams and organisations are less complex. In making explicit some of the assumptions about how leaders lead integrated care teams and systems this review has contributed significant new perspectives, offering fresh theoretical grounding that can be built on, developed and tested further. Feasibility and Effectiveness of Parent–Child Interaction Therapy with Victims of Domestic Violence: A Pilot Study

Parent-Child Interaction Therapy (PCIT) is an evidence-based treatment for young children (aged 2.5 to 7 years) with externalizing behavior problems. Since its development, PCIT has been applied to a wide array of childhood problems and has a significant evidence base for families with histories of child physical abuse. The current study extended the existing literature by testing the effectiveness and feasibility of PCIT in an urban domestic violence shelter with community-based clinicians delivering the treatment. Seven clinicians implemented PCIT with parent-child dyads which included 21 preschool (M = 4.57 years; SD = 1.50) children. Families completed assessments at baseline, mid-treatment, and post-treatment. Nine families completed PCIT (43%). Completion of PCIT was associated with improved child behavior, parenting practices, and mental health symptoms. Considerations for treatment delivery and future directions are discussed.

Shelter Versus Living with Abusive Partner: Differences Among Mothers and Children Exposed to Intimate Partner Violence

A growing body of literature has examined differences in the mental health and social support of abused women living in shelters compared to women from the community. However, less attention has been given on both mothers and children. A crosssectional study was carried out to examine differences on cortisol awakening response (CAR), depression, anxiety and post-traumatic stress symptoms, and social support in women living in shelter compared to women living with abusive partner. They also reported on their children's internalizing and externalizing symptoms. The sample included 162 mother-child dyads, which 81 were living with abusive partner and 81 were living in shelters. Mother' ages ranged from 21 to 54 years (M = 36.39, SD = 7.64) and children' age ranged from four to 10 years old (M = 7.23, SD = 1.94), 75 (46.3%) females and 87 (53.7%) males. After adjusting for potential effects of covariates, mothers living in shelters presented better adjustment in terms of psychological, physiological and social functioning. Social support was the only variable with a large effect size, while the others showed only small effects. In the case of children, those living in shelters displayed higher levels of internalizing and externalizing symptoms compared to children living at home with their mothers and abusive partner. This study suggest that for women, shelter residence may be helpful for mental health and for significant improvements in perceived social support, but for children, the results seem to suggest that they are not benefiting from the time they spend at shelter. Future studies using samples from battered women's shelters need to evaluate if the services offered are suitable for children´s needs.
