Vacuum phenomenon in osteoarthritis of the atlanto-odontoid joint: CT findings

Abstract: Out of 620 consecutive examinations of the atlanto-odontoid joint in patients referred for computed tomography (CT) of the brain or paranasal sinuses, gas collection within the atlanto-odontoid joint was observed in 12 (1.9%). Degenerative abnormalities at the atlanto-odontoid joint were significantly more frequent in patients with a gas-induced vacuum phenomenon than in an age-matched control group. The CT findings and association of the vacuum phenomenon with osteoarthritis of the atlanto-odontoid joint are … Show more

“…Similar to other synovial joints, osteoarthritis of the atlanto-dental joint involves various changes, including narrowing of the joint space, marginal cortical thickening, ligamentous calcification, and vacuum phenomenon (1)(2)(3)(4), and these changes are displayed more clearly by CT than by a lateral cervical radiographic projection (5). At radiography or autopsy, 80-88% of atlanto-dental joints in adults aged >60 years old were degenerative, and osteoarthritis appeared after 30 years of age (6).…”

“…The presence of gas within a joint space was first described by Fick in 1910, and subsequently, it was reported in the intervertebral disc, pubic symphysis, peripheral synovial joints with open injury, with the traction of the joints as in hip, with infective spinal causes, non-open injury as in ankle and temporomandibular joint (TMJ), and mostly with degenerative changes in all the joints [2][3][4][5][6][7][8]. Although routine radiographs have been mainly used to demonstrate vacuum phenomenon, other modalities such as ultrasonography (USG), CT scan (CT), and MRI can better delineate the air shadow.…”
