Vagal impairment in elderly Chagas disease patients: A population-based study (The Bambuí Study)

Abstract: Disease-specific vagal cardiac dysfunction was observed in ChD individuals below 70. However, further ageing interferes significantly with vagal heart modulation, attenuating the difference of HRV indexes between ChD and non-ChD subjects.

“…However, a sample of elderly chagasics with the indeterminate form of the disease did not showed any alterations in cardiac autonomic function as compared with healthy elderly subjects 40 . Contrasting with this finding, elderly chagasics below 70 years, but not those older, with an uncharacterised form of the disease, show reduced vagal modulation 41 .…”

“…Depressed sensitivity of the baroreflex mechanism resulting in blunted bradycardia or tachycardia in response to acute elevation or reduction of arterial pressure has been observed 32,77 , as have altered baroreflex responses traduced by deficient heart rate responses during and after the Valsalva manoeuvre in chagasics with any form of the disease 8,33,36,78,79 . Additionally, moment-to-moment heart rate variability is variably depressed in chagasic subjects both in supine and standing positions 9,10,39,41,46,68,80 . These autonomic cardiovascular disturbances occur most often in chagasics with exclusive or combined heart disease.…”

“…Nonetheless, more recent studies have shown that the clinical manifestations of ChD in old ages are similar -or even worse -than those observed in young adults [9][10][11][12]. Indeed, an analysis of mortality and public hospitalization rates in Brazil in the last decades showed that a substantial toll of the burden of T. cruzi infection affected older adults [4], suggesting that ChD in the elderly might be more severe and potentially lethal than suspected in earlier studies.…”

“…Some of the less known HRV indices as well as the more common ones have been used to investigate autonomic dysfunction: RSA 5RRmc for assessing diabetes [53], Guillain-Barré syndrome [54], and sinus node dysfunction [55]; RMSSD for detecting vagal impairment in Chagas disease in 10 s [56]. Various cardiac conditions have been investigated with HRV.…”
