Vagal Paraganglioma: Surgical Removal with Superior Laryngeal Nerve Preservation

Abstract: Vagal paragangliomas (VPGLs) represent <5% of all head and neck paragangliomas (PGLs) and show a 17% to 20% risk of malignancy. We present a rare case of a 50-year-old gender with a left VPGL in her neck. To date, approximately 200 cases have been reported. The tumor showed web-like adhesions and arterial supply from the external carotid artery. We performed en bloc resection including a part of the vagus nerve. The superior laryngeal nerve was preserved with the “human communicating nerve” which maintains neu… Show more

“…CT, CT angiography, MRI, and PET/CT have been found useful for the diagnosis of the disease [ 1 , 2 ]. Specifically, Ga-labelled somatostatin agonist PET is favored regardless of the patient's genetic background [ 13 ]. Fine-needle biopsy plays an elusive role in the diagnosis of paragangliomas, and cytology is non-diagnostic most of the time [ 2 , 6 ].…”

“…Conventionally, apart from radionuclide therapy administration of alkylating agents or tyrosine kinase inhibitors (13), the nonsurgical treatment of paragangliomas entails recent methods of radiotherapy, which involve single-fraction gamma knife radiosurgery and CyberKnife (Accuray Incorporated, Sunnyvale, CA) radiosurgery [8,9,13].…”

“…Paragangliomas(PGLs) are neoplasms of neural crest origin.PGLs are rare in head and neck region.Even thoughdifficult to estimate the exact incidence of head and neck PGLs," they represent <0.5% of all head and neck tumours. They couldbe originated from the carotid body, tympanic, jugular or vagal" paraganglia 1 .…”

“…They range from pulsatile tinnitus/ringing in the ear to deficits of cranial nerves (such as hoarseness and dysphagia) and intracranial extension associated with an increased risk of death [ 5 ]. VPGL accounts for approximately 13% of all head and neck paragangliomas (HNPGLs) and occurs more frequently in women [ 6 , 7 ]. Surgical resection is a primary treatment for VPGL, whereas radiation therapy is used in case of malignant and unresectable tumors [ 8 , 9 ].…”
