Vaginal Hysterectomy for Benign Disease Without Prolapse

Abstract: Vaginal hysterectomy was the first minimally invasive surgical approach for benign gynecologic conditions. Recently a number of other approaches for hysterectomy have been introduced, yet when compared with vaginal hysterectomy these approaches do not offer significant benefit for similar indications. The focus of this chapter is to provide comprehensive review of vaginal hysterectomy for the practicing clinician.

“…14 There are many approaches to hysterectomy for benign diseases, including AH, VH, laparoscopic assisted vaginal hysterectomy (LAVH), total laparoscopic hysterectomy (TLH), and subtotal laparoscopic hysterectomy. With the constant modernization of minimally invasive concepts in obstetrics and gynaecology, doctors choose surgical routes by considering not only the patient's health status, but also the psychological needs of patient and the patient quality of life after surgery.…”

“…10 Since its advent, laparoscopic surgeries have been criticized as it was a difficult procedure which could be performed only by experienced surgeons. The duration of the surgery also was prolonged compared the other types, but the duration of the hospital stay was far less and the post-operative pain also was comparatively lesser.…”

“…Vaginal hysterectomy has comparable outcomes with laparoscopic hysterectomy, except for a shorter operative time. However, previous caesarean delivery, nulliparity or no prior vaginal delivery, large uterus, lack of uterine descent and planned oophorectomy, are relative contraindications for vaginal hysterectomy (Kulkarni and Rogers 2010). Although, the benefi ts of vaginal hysterectomy are recognised, many gynaecologists are uncomfortable with this technique.…”
