Vaginal leiomyoma presenting as a lateral vaginal wall mass

Abstract: Leiomyomas are classified as benign mesenchymal neoplasms and consist of smooth muscle cells with variable amounts of fibrous stroma. The tumours occur most frequently in the uterus, affecting 20-30% of women of reproductive age but vaginal leiomyomas are rare with only around 300 cases reported since the first case was described in 1733. These tumours are thought to arise from Müllerian smooth muscle cells in the sub-epithelium of the vagina. Vaginal leiomyomas are usually situated in the anterior vaginal wal… Show more

“…The treatment of choice for vaginal leiomyoma is surgical excision and enucleation as it confirms the diagnosis and also rules out the possibility of malignancy. It can be done either through vaginal or abdominal-perineal route depending upon the size of the mass [7]. Three patients showed recurrence within 2 years in a case series presented by Zhao's et al, thus encouraging the need of long-term follow-up for patients with vaginal leiomyoma [8].…”

“…Vaginal leiomyomas appear as well-defined solid masses with low signal intensity with homogenous contrast enhancement on T1-and T2-weighted images in magnetic resonance imaging. However, the gold standard remains histopathology for the diagnosis [7]. The treatment of choice for vaginal leiomyoma is surgical excision and enucleation as it confirms the diagnosis and also rules out the possibility of malignancy.…”

“…Vaginal leiomyoma is usually firm but, may undergo degeneration to become soft. The variation in the consistency from firm to soft makes diagnosis more difficult (3,6). It is usually asymptomatic until it reaches a larger size (4, 7).…”

“…In most cases of vaginal leiomyomas, there is no change in the vaginal mucosa. When a vaginal tumor is associated with ulceration, bleeding, or purulent discharge, the possibility of malignancy should be considered [ 1 , 3 ].…”
