Validation of International Chronic Ocular Graft-Versus-Host Disease (GVHD) Group Diagnostic Criteria as a Chronic Ocular GVHD-Specific Metric

Abstract: The newly proposed ICCGVHD diagnostic criteria can be used reproducibly for the diagnosis and determination of severity of chronic ocular GVHD. However, larger prospective studies are needed to further validate it.

“…65 Those subjective and objective variables are used worldwide for diagnosing dry eye. Three studies from ours and other institutes validated the newly proposed criteria and showed good agreement between the international chronic ocular GVHD Consensus Group scores (ICOS) and some clinical parameters, including tear film osmolality, 94 best clinical practices, 95 and immune cell and corneal sub-basal nerve abnormality by in vivo confocal study. 96 One study showed only a weakly positive correlation with subepithelial fibrosis and severe dry eye measured by using ICOS, although there is no significant correlation among subepithelial fibrosis, meibomian gland atrophy, and ICOS.…”

“…A recent study validated for the first time the ICCGVHD criteria, comparing them with best clinical practice (BCP), defined as ocular GVHD evaluation by highly trained single expert in ophthalmology on the basis of comprehensive clinical observation of the tear film and meibomian glands, among others [20]. There was slight overall agreement between BCP assessment and the ICCGVHD criteria for the diagnosis of chronic ocular GVHD.…”

“…A study comparing diagnostic utility between the 2005 NIH criteria and the ICOGCG criteria showed that the more stringent ICOGCG criteria better differentiated ocular GVHD [35]. Validation of ICOGCG diagnostic criteria, in comparison with diagnosis based on best clinical practice, showed good agreement and reproducibility, especially in more severe cases of ocular GVHD [54]. Further validation of the ICOGCG score, particularly for the applicability in clinical practice and as a response measure, is needed.…”
