Validation of original and alternative fistula risk scores in postoperative pancreatic fistula

Abstract: BackgroundIn 2013, the fistula risk score (FRS) was developed to assess the risk of clinically relevant postoperative pancreatic fistula (CR‐POPF). In 2017, the alternative FRS (a‐FRS) was proposed. The purpose of this study was to validate the original FRS (o‐FRS) and a‐FRS for CR‐POPF in pancreaticoduodenectomy (PD).MethodsFrom January 2007 to December 2016, 1,771 patients underwent PD for periampullary cancers. POPF was defined and classified according to the 2016 International Study Group for Pancreatic Fi… Show more

“…However, the new criteria that do not include the DFA markedly reduced the incidence of CR-POPF as well as the incidence of the Clavien-Dindo grade ≥ 3 complications in both the validation and further assessment cohorts. Numerous attempts to reduce CR-POPF have been reported [7][8][9][10][11][12][13][14][15]20,23 , and these studies revealed the proper management of prophylactic drain(s) is the most important issue. Intra-abdominal infection was strongly associated with the development of CR-POPF 9,10,[11][12][13][14]22 .…”

“…Some studies believe that the influencing factors of pancreatic fistula after splenectomy are mainly related to the grading of liver function, blood coagulation mechanism, the texture of pancreas, the size of spleen, the degree of hypersplenism, and the operation method. [14][15][16] In the present work, we elucidate and analyze the following three representative influencing factors. 1) Grading of liver function and blood coagulation mechanism: The impairment of liver function is also related to the blood coagulation mechanism of patients, which is affected by intraoperative clamping and, meanwhile, increasing hemostasis by ligation that easily injure pancreatic tissue.…”

“…Numerous risk factors for POPF have been identified, such as sex, pancreatic texture, blood loss, and main pancreatic duct diameter; however, most of these predictors are subjective and contradictory [ 10 ]. Several predictive models have also been proposed, among which the fistula risk score and alternative fistula risk score (a-FRS) are most commonly used [ 9 , 10 ]. However, the performances of those models were recently suggested to have low predictive value for external validation [ 9 , 10 , 14 , 15 ] .…”
