ObjectiveThis study aims to evaluate the efficacy of chemoradiotherapy (CRT) in cervical cancer using diffusion-weighted imaging (DWI) with apparent diffusion coefficient (ADC) values.MethodsA total of 71 patients with cervical cancer were enrolled in this study. All patients underwent conventional magnetic resonance imaging and DWI scanning before CRT and at 7, 14, 21 days, and 6 months after CRT. These patients were divided into the complete response (CR) and non-CR groups according to the response evaluation criteria in solid tumors criteria. Receiver operating characteristic (ROC) curve was used to evaluate the accuracy of ADC values in predicting the efficacy of CRT in cervical cancer.ResultsCompared with before-CRT treatment, tumor volumes were reduced and ADC values were elevated in both CR and non-CR groups after CRT treatment. At 21 days after CRT, tumor volumes in the CR group were smaller than those in the non-CR group. During the period of 21 days to 6 months after CRT, tumor regression rate and the increased rate of ADC values in the CR group were higher than those in the non-CR group. ROC curves revealed that the increased rate of ADC values at 21 days after CRT was the optimal time point for the prediction of CRT efficacy in cervical cancer, with the area under the curve, sensitivity, and specificity of 0.775, 92.7%, and 62.5%, respectively.ConclusionOur study provides evidence that the increased rate of ADC at 21 days after CRT might be a promising tool for predicting the efficacy of CRT in cervical cancer.

Backgrounds/Aims. Watson for Oncology (WFO) is a cognitive technology that processes medical information by analyzing the latest evidence and guidelines. However, studies of the concordance rate between WFO and clinicians for advanced gastric cancer (AGC) are lacking. Methods. We retrospectively reviewed 65 patients with AGC who consulted WFO and the Gachon Gil Medical Center multidisciplinary team (GMDT) in 2016 and 2017. The recommendations of WFO were compared with the opinions of the GMDT. WFO provided three treatment options: recommended (first treatment option), for consideration (second treatment option), and not recommended. Results. In total, 65 patients (mean age 61.0 years; 44 males and 21 females) were included in the study. The concordance rate between WFO and the GMDT was 41.5% (27/65) at the recommended level and 87.7% (57/65) at the for consideration level. The main causes of discordance between WFO and the GMDT were as follows. First, WFO did not consider the medical history. Second, WFO recommended the use of agents that are considered outdated in Korea. Third, some patients wanted to be involved in a clinical trial. Fourth, some patients refused to use the biologic agents recommended by WFO for financial reasons as they were not covered by medical insurance. Conclusions. The concordance rate at the recommended level was relatively low but was higher at the for consideration level. Discordances arose mainly from the different medical circumstances at the Gachon Gil Medical Center (GMC) and the Memorial Sloan Kettering Cancer Center (MSKCC), the main WFO consulting center. The utility of WFO as a tool for supporting clinical decision making could be further improved by incorporating regional guidelines.

BackgroundEndoscopy is the main approach used for esophageal squamous cell carcinoma (ESCC) screening, especially in high-risk areas. However, little consensus has been achieved in recent ESCC screening programs, and endoscopists have selected patients only by age and family history.Patients and methodsTo generate a proper strategy for selecting an eligible population for endoscopic screening based on demographic factors, lifestyle, and eating habits, a total of 7,830 residents in an area with a high risk of ESCC were recruited for this study. All participants underwent endoscopic examinations that were conducted by experienced endoscopists. Risk factors for ESCC and other lesions were selected by univariate and multivariate logistic regressions. A nomogram for the prediction of ESCC and premalignant lesions was constructed, which included information on age, sex, occupation, labor intensity, income, and mining exposure. Receiver operating characteristic (ROC) curve analysis was performed to present the predictive accuracy of the nomograms.ResultsThe area under the curve (95% CI) was 0.749 (0.711–0.788) for this nomogram. By applying this nomogram, we could exclude 60% (4704/7830) of patients before endoscopy screening and detect all ESCC cases as well as most esophageal lesions in the remaining population.ConclusionIn conclusion, we provided a ready-to-use preclinical tool with the potential to select eligible people with high risk of ESCC for endoscopy screening.
