Validation of the PAGE-B score to predict hepatocellular carcinoma risk in caucasian chronic hepatitis B patients on treatment

Abstract: BACKGROUND Several risk scores have been developed to predict hepatocellular carcinoma (HCC) risk in chronic hepatitis B (CHB) patients. The majority of risk scores are based on pretreatment variables that are no longer considered risk factors for HCC development due to the suppression of hepatitis B virus replication early in the course of potent antiviral treatment in most patients. “…et al(2022)[62] encontraram incidência cumulativa de CHC em 3 e 5 anos através do PAGE-B, de 0% e 0% para baixo, 1,2% e 1,5% para moderado e 11,7% e 12,5% para alto risco respectivamente (log-rank p< 0,001), aproximando-se deste estudo apenas a previsão do alto risco em 5 anos.Porém, na população estudada por eles, incluíram-se apenas os que estavam em tratamento medicamentoso e os que possuíam cirrose descompensada. Na atual análise, quatro pacientes não possuíam cirrose ao diagnóstico de ICVHB e evoluíram para CHC com escores de intermediário e alto risco (exceto um caso classificado como baixo pelo mPAGE-B já discutido anteriormente), similar aos 11 pacientes com cirrose ao início, demonstrando que os escores podem ter boa predição em pacientes independente do grau de fibrose hepática, necessitando de estudos com maior número de pacientes para validação dessa hipótese.O estudo apresenta algumas limitações.…”
