Background and Objective Several medication adherence patient-reported outcome measures (MA-PROMs) are available for use in patients with cardiovascular disease (CVD); however, little evidence is available on the most suitable MA-PROM to measure medication adherence in patients with CVD. The aim of this systematic review is to synthesise the measurement properties of MA-PROMs for patients with CVD and identify the most suitable MA-PROM for use in clinical practice or future research in patients with CVD. Methods An electronic search of nine databases (PubMed, MEDLINE, CINAHL, ProQuest Health and Medicine, Cochrane Library, PsychInfo, Scopus, Embase, and Web of Science) was conducted to identify studies that have reported on at least one of the measurement properties of MA-PROMs in patients with CVD. The methodological quality of the studies included in the systematic review was evaluated using the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) checklist. Results A total of 40 MA-PROMs were identified in the 84 included studies. This review found there is a lack of moderate-to-high quality evidence of sufficient content validity for all MA-PROMs for patients with CVDs. Only eight MA-PROMs were classified in COSMIN recommendation category A. They exhibited sufficient content validity with very low-quality evidence, and moderate-to-high quality evidence for sufficient internal consistency. The 28 MA-PROMs that meet the requirements for COSMIN recommendation category ‘B’ require further validation studies. Four MA-PROMs including Hill–Bone Compliance Medication Scale (HBMS), the five-item Medication Adherence Report Scale (MARS-5), Maastricht Utrecht Adherence in Hypertension (MUAH), and MUAH-16 have insufficient results with high quality evidence for at least one measurement property and consequently are not recommended for use in patients with CVD. Two MA-PROMs (Adherence to Refills and Medications Scale [ARMS] and ARMS-7) are comprehensive and have moderate to high quality evidence for four sufficient measurement properties. Conclusion From the eight MA-PROMs in COSMIN recommendation category A, ARMS and ARMS-7 were selected as the most suitable MA-PROMs for use in patients with CVD. They are the most comprehensive with be best quality evidence to support their use in clinical practice and research. Adherence to treatment is of utmost importance for the management of patients with cardiovascular disease. The present study aimed to determine the degree of adherence to treatment and its relationship with anxiety and depression in patients with cardiovascular diseases. Materials and Methods: A cross-sectional study was conducted among patients with cardiovascular disease admitted to teaching hospitals in Qom, Iran in 2016. One hundred sixteen patients who met the inclusion criteria were selected via the convenience sampling method. Data collection instruments included demographic information form, Adherence to Refills and Medications Scale, as well as Hospital Anxiety and Depression Scale. The data were analyzed in SPSS software using descriptive tests and linear regression analysis. Results: Based on the obtained results, about half of the patients (49.1%) scored lower than average in adherence to treatment. The results of multiple linear regression analysis showed that among the variables included in the univariate model (P<0.15), depression (P=-0.26; β = 0.047) and body mass index (BMI) (P=0.22; β = 0.26) were significantly correlated with treatment adherence. Conclusion:As evidenced by the results of the current study, patients' adherence to a medication regimen was associated with depression and BMI. Psychological problems, especially depression, deserve assiduous attention as important determinants of treatment adherence.

Background: Medication adherence is essential to achieving controlled blood sugar in diabetic patients. Insulin generally provides better glycemic control but is considered painful and requires special techniques. Insulin administration in patients with neurological complications requires particular consideration because these complications can cause physical and cognitive barriers. Objective: This study analyses the effect of insulin administration on medication adherence in diabetic patients with neurological complications and the influence of various confounding variables (baseline characteristics, medical and medication history). Methods: This observational study was conducted with a cross-sectional design at a government hospital in East Jakarta from September 2021 to January 2022. The sample was type 2 diabetes mellitus patients with neurological complications who received antidiabetics for at least six months. The neurological complications include central nervous disorders (such as stroke) and peripheral nervous disorders (such as neuropathy). The independent variable was insulin administration, while the dependent variable was adherence, measured using subjective methods [Adherence to Refills and Medications Scale (ARMS)] and objective methods (Medication Refill Adherence (MRA) and HbA1c]. Results: Of 175 respondents, based on the three methods (MRA, ARMS, HbA1c), 13 respondents (7.4%) were adherent, namely one respondent (1.8%) in the insulin group and 12 respondents (10.1%) in the non-insulin group. Insulin administration affects adherence to antidiabetics by 0.123 times (95% CI: 0.015 - 1.024), or patients who use insulin have 87.7% lower adherence controlled by antidiabetic changes and the total number of medicines used. Conclusion: Insulin administration significantly affects medication adherence in diabetes mellitus patients with neurological complications.
