Validity of Pneumonia Severity Index and CURB-65 Severity Scoring Systems in Community Acquired Pneumonia in an Indian Setting

Abstract: Background. Little information is available from India regarding prognostic factors in patients with community acquired pneumonia (CAP). Methods.Hospital-based prospective study to test the validity of pneumonia severity index (PSI) and the confusion, urea, respiratory rate, blood pressure, age over 65 years (CURB-65) risk scoring systems in patients with CAP (n=150).Results. Although both CURB-65 class ≥III and PSI class ≥IV were 100% sensitive in predicting death, CURB-65 class ≥III had a higher specificity … Show more

“…Most patients included in our study had a CURB-65 score of 3 or more which justifies their admission in ICU. 6 A microbiological diagnosis was achieved in 96% of the patients in the study. The total yield of isolation of microbes in the sputum may vary from 34% to 86% as reported in National Pneumonia Guidelines.…”

“…CURB-65 (Confusion, urea, respiratory rate, blood pressure plus age ≥65 year) score has been found to be helpful in deciding the need for admission and site of treatment in pneumonia patients. 6 The microbiology may be different in patients of community-acquired pneumonia (CAP) who have been previously hospitalised within 90 days of present illness. 7 Thus, it is important to differentiate between CAP and HCAP (health care associated pneumonia).…”

“…Liver dysfunction is a possible cause of persistent lung damage. Indeed, liver failure plays a significant role in individuals with bacterial chest sepsis[ 75 ]. Cirrhosis and SARS-CoV-2 may have a fatal synergy because of alterations in the immune system caused by viral infection and coagulation.…”

“…Yoğun bakım ünitesine yatış oranının literatüre göre biraz daha fazla olması ilimizde tek üçüncü basamak hastane olmamız nedeniyle ağır olguların hastanemize yönlendirilmesiyle açıklanabilir. TGP'lerin ağırlık derecesini saptamak için çok sayıda skorlama sistemi kullanılmaktadır (14)(15)(16). Ülkemizde en çok CURB-65 ve pnömoni şiddet indeksi (PSI) kullanılmaktadır.…”
