Validity of the EQ-5D-5L and EQ-5D-3L in patients with Crohn’s disease

Abstract: This is the first study to report the impact of CD on quality of life using the EQ-5D-5L questionnaire. The 5L seems to perform better than 3L in terms of feasibility, ceiling effect, discriminatory power and known-groups validity. Understanding the differences in psychometrics between the 3L and 5L is essential as they have substantial implications for financial decision-making about CD treatments.

“…We suggest considering the use of the EQ‐5D‐5L version (with five‐level response scale) to explore sHE in future studies [30]. The EQ‐5D‐5L has been proved to have better measurement properties as a health status measure than the EQ‐5D‐3L [10], and we expect an increase in its use in CD registries and clinical studies. We used the tariffs of the UK to calculate actual and expected EQ‐5D‐3L scores due to lack of local tariffs at the time of the analyses.…”

“…In terms of studies comparing the measurement properties of 3L and 5L versions of EQ-5D, many studies have been carried out across other populations [16]. Most of these studies showed that the 5L is highly consistent with 3L responses, as well as offering a better level of performance in terms of reduced ceiling effects and better informativity compared to the 3L [16][17][18]. Ceiling effects occur when a high proportion of subjects have maximum scores on the measurement of interest.…”

“…As indicated by Buchholz et al [27], most of the study populations are disease specific: inpatient rehabilitation [29], orthopaedics [30,31], stroke [32,33], liver disease [34][35][36], cancer [37], lung cancer [38], diabetes [39,40], dermatology [41], psoriasis [42] and mixed population [43]. Recently published studies comparing EQ-5D-3L and EQ-5D-5L also refer to the disease-specific subpopulations: cataract [44], Crohns disease [45] and hip replacement surgery [46]. However, disease-specific populations characterise by predictively worse health and resulting HRQoL, so the distribution of assessed health states in these groups is shifted compared to the distribution in the general population.…”
