Aims Valsalva retinopathy produces sudden visual loss, which may be prolonged if untreated. Nd:YAG laser enables rapid diffusion of premacular subhyaloid haemorrhage. This study was performed to assess the long-term results and safety of Nd:YAG laser treatment in cases with Valsalva retinopathy. Methods Sixteen patients had Nd:YAG laser treatment to drain premacular haemorrhage. The follow-up period was 24 months. Results All eyes had marked clearing of haemorrhage and immediate improvement of vision following laser treatment. In 14 eyes visual acuity improved to 20/20 level at the end of the first week and the remaining two patients achieved 20/20 level within 1 month. No patient had evidence of retinal or choroidal damage. Conclusion Nd:YAG laser treatment for Valsalva retinopathy is an effective, noninvasive, and safe procedure for patients with a premacular subhyaloid haemorrhage larger than 3 disc diameter and no longer than 3 weeks of duration.

W pracy omówiono wpływ podwyższonego ciśnienia atmosferycznego (kompresji i dekompresji w komorze hiperbarycznej) na organizm ludzki, w szczególności na narząd wzroku. Przedstawiono dość rzadki przypadek bezbolesnej utraty widzenia związany z wylewem siatkówkowym i następowym bliznowaceniem u osoby wykonującej próbę Valsalvy podczas pracy w komorze hiperbarycznej. Zmianę określono z dużym prawdopodobieństwem jako następstwo makulopatii Valsalvy. Przypadek analizowano-na wniosek pacjentki-po około 6 latach od wystąpienia choroby pod kątem istnienia lub braku podstaw do rozpoznania choroby zawodowej narządu wzroku.

A 54-year-old healthy male presented complaining of sudden loss of vision in the right eye. Initial visual acuity was counting fingers. The patient's acute vision loss developed after sexual activity. Color fundus photos and fluorescein angiography were performed showing a large subinternal limiting membrane hemorrhage in the macular area. A 23-gauge sutureless pars plana vitrectomy with brilliant blue assisted internal limiting membrane peeling was performed with best-corrected visual acuity recovery to 20/50 at 6 months of followup.
