Vascular dynamics in isolated systolic arterial hypertension

Abstract: Summary:With a growing elderly population, the incidence ol' isolated systolic hypertension (ISH) has increased. This study characterizes dynamic vascular changes that occur with achnnced age and with ISH. Fifty-the healthy individuals and seven with ISH were distributed in seven age groups from the second to the seventh decade. An index of aortic stiffness (AWAV) was derived using a mercury sphygmomanometer to oht;lin pulse pressure, and ultrasonographic measurements were iiaed to estimate aortic volumes appl… Show more

“…Whether the rise in isobaric arterial compliance was caused by a higher number of open parallel vessels or by a decrease in individual vessel wall tone in the ISH patients could not be ascertained in this study. Nevertheless, the resulting increase in arterial blood volume agrees well with the higher blood volumes (table 2) and arterial dilatation suggested by others in elderly patients with ISH [6, 15, 22]. …”

“…For this purpose, a noninvasive, electrical bioimpedance method [14]was used, which provides information complementary to that of previously used methods in ISH research. Instead of the usual, dynamic compliance of a single pulsatile artery [13, 15, 16, 17], it yields the static compliance of the upper arm arterial bed as a whole. Furthermore, the arterial distensibility, the ratio of arterial compliance to arterial volume and some venous characteristics are determined.…”

“…36 However, only 1 has previously reported increased aortic PWV in ISH, and that difference did not persist after correcting for differences in MAP. 37 In the present study, aortic PWV was increased in the ISH group at ambient pressures compared with matched, normotensive controls, and this difference persisted even after adjusting for the slightly higher MAP in subjects with ISH (Ϸ17% higher).…”

“…The maximum and minimum diameters (d max and d min ) were measured by bidimensional images taking the same references used for the aorta. Aortic and carotid stiffness (␤) was calculated using the formula previously validated at our institution [␤ ‫ס‬ 127.32 (SP − DP)/(d max 2 − d min 2)] [1,6].…”
