Clavicular reconstruction is typically managed conservatively. Despite demonstrating improving outcomes, including range of motion and pain, there are currently no published reports of acute reconstruction with vascularized free fibula flaps (VFFF) or vascularized periosteal extensions in adult patients with clavicular defects. VFFFs have been utilized to correct critical bony defects of the clavicle and chronic nonunions; however, descriptions following acute trauma are rare. Bony union enhancement with periosteal extension has been described in both pediatric and adult populations, but never in the case of clavicular reconstruction. Herein, we seek to fill this gap in the literature by describing the acute reconstruction of a 6.5 cm bony gap in a 29‐year‐old male following a gunshot wound to the chest, utilizing a 6.5 cm VFFF with periosteal extension, and inset to the internal mammary vessels. The postoperative course was uncomplicated, with bony consolidation noted by 10 weeks, and full, pain‐free range of motion at 8 months, showing this technique may be a viable option following acute trauma.

Introducción: el tumor de células gigantes representa de 10 a 12% de los tumores benignos de comportamiento agresivo con capacidad de invasión local o a distancia, misma que incrementa si se trata de un grado III de la clasificación de Campanacci. Estadio en el que se recomienda realizar resección en bloque, dado su alto grado de recurrencia. Debido al alto grado funcional que presenta esta región, la reconstrucción del radio distal continúa siendo un desafío y aún no existe una técnica definida como estándar de oro, por lo que se han desarrollado múltiples técnicas quirúrgicas, dentro de las cuales los colgajos vascularizados continúan siendo de las mejores opciones sobre todo si se trata del peroné; sin embargo, se han descrito múltiples complicaciones de éstas, la que se presenta con mayor frecuencia es la no unión; pero no hay estudios enfocados en el análisis de su tratamiento. En esta ocasión describimos una técnica modificada para la toma de un colgajo vascularizado perióstico cubital pediculado proveniente de la arteria interosea anterior, con abordaje dorsal para el tratamiento de la no unión de radio, después de una reconstrucción con peroné vascularizado. Objetivo: mostrar los resultados clínicos y radiográficos del uso del colgajo vascularizado perióstico cubital pediculado (CVPCP) proveniente de la arteria interósea anterior (AIA). Además de describir la técnica quirúrgica modificada a través de un abordaje dorsal en una paciente con no unión de radio. Presentación del caso: paciente de 45 años de edad con antecedente de tumor de células gigantes (TCG) de radio distal grado III de Campanacci tratada con resección en bloque y reconstrucción inmediata con peroné vascularizado que evolucionó con una no unión, que fue tratada con la colocación de un CVPCP con una evolución satisfactoria clínica y radiográficamente. Conclusión: el CVPCP puede considerarse una opción confiable y útil para el tratamiento de complicación post-reconstrucción en antebrazo en adultos.Palabras clave: tumor de células gigantes, no unión, reconstrucción de muñeca, colgajo perióstico vascularizado, arteria interósea anterior.

Symptomatic Heterotopic Bone Formation after 1,2 ICSRA in Scaphoid Nonunions

Background We observed several cases of heterotopic bone formation after a 1,2 intercompartmental supraretinacular artery (1,2 ICSRA) distal radius vascularized bone graft (VBG) for the treatment of scaphoid nonunion. This adverse event seems underreported. Knowledge about factors associated with the formation of heterotopic bone after VBGs might help reduce this adverse event. Purpose What factors are associated with resected heterotopic bone formation after 1,2 ICSRA distal radius graft for the treatment of scaphoid nonunion? Patients and Methods We retrospectively reviewed all patients with a scaphoid nonunion treated with a 1,2 ICSRA distal radius graft between 2008 and 2019 in an urban level 1 trauma center in the Netherlands. We included 42 scaphoid nonunions in 41 people treated with the 1,2 ICSRA graft. We assessed potential correlation with patient, fracture, and treatment demographics. Results Heterotopic bone developed in 23 VBGs (55% [23/42]), of which 5 (12% [5/42]) were resected. Heterotopic bone was located radially (at the pedicle side) in all participants. Except a longer follow-up time (p = 0.028), we found no variables associated with the development of heterotopic bone formation. Conclusion The location of the heterotopic bone at the pedicle site in all cases suggests a potential association with the periosteal strip. Surgeons might consider not to oversize the periosteal strip as a potential method to prevent heterotopic ossification after VBG. Level of Evidence Level II, prognostic study.
