Venous glucose, serum lactate and base deficit as biochemical predictors of mortality in patients with polytrauma.

Abstract: BACKGROUND:The trauma and injury severity score (TRISS) and Acute Physiology and Chronic Health Evaluation IV (APACHE IV) are accurate but complex. This study aimed to compare venous glucose, levels of serum lactate, and base deficit in polytraumatized patients as simple parameters to predict the mortality in these patients versus (TRISS) and (APACHE IV).

“…13 Random blood sugar measurement in polytrauma patient was missed in most of studies or had low application percentage .Although, It is have a high importance in prognosis and easy rapid biochemical predictors of mortality in patients with polytrauma. 14 Our study showed that that pelvic stability assessed in (65%) of cases which agree with Spanjersberg et al in which pelvic stability assessed in (64.8%) and Tsang et al, (65.6%). 9,11 And about 2 years survey, our study showed that for majority of cases head to toe examination was done percentage (78.6%), while AMPLE history was taken in minority of cases percentage (33%) ( Table 6).…”

“…The main strength of this study are the strict matching criteria which all are proven and validated predictors of outcome in trauma (20,(30)(31)(32)(33).…”

“…Furthermore, some of the matching criteria require critical discussion. Even though GCS, coagulopathy, BE and ISS are considered to be strong predictors for adverse outcome in trauma (29), elderly patients are presenting with more comorbidities, hence there might be a bias within the matching itself due to pre-existing conditions (20,(30)(31)(32)(33). It is more likely to nd a pre-existing decreased GCS in an older patient.…”

“…In the <40 age group, its incidence is 44.9% and it is the most common cause of death. Th e mortality and morbidity rates and functional recovery are directly aff ected by professional and timely care 2,3 . Appropriate triage and optimization of treatment approach in such patients is one of the key factors that aff ect fi nal outcome in these patients 1,6,7 .…”

“…Patients with polytrauma are one of the greatest challenges faced by physicians 1 . Polytrauma is a term describing patients with multiple injuries that involve several parts of the body and compromise function of the body and/or organ involved [1][2][3] . Th e most popular and most commonly used defi nition of trauma is based on the Injury Severity Score (ISS) 2,4,5 .…”
