Ventilator Management for Neuromuscular Disease

Abstract: Many progressive neuromuscular disorders involve the muscles of the respiratory system, and respiratory insufficiency is a major cause of morbidity and mortality. Thus ventilatory assistance becomes an important part of disease management for patients with advanced neuromuscular disease. The following discusses the evaluation of patients with neuromuscular disease to characterize the nature and severity of respiratory muscle involvement, indications and contraindications for the use of noninvasive ventilation,… Show more

“…IPAP is typically increased by 2 -3 cm of H 2 0 as needed to improve symptoms. This is in contrast to some experts who strongly recommend using ''high span'' NIPPV for all ALS patients, defined as greater than 10 cm H 2 0 difference between inspiratory and expiratory pressures, with an average IPAP of 16-18 cm H 2 0 [9,10].…”

“…Involvement of the upper airway musculature may produce additional symptoms, such as difficulties with speech or swallowing, and aspiration associated with dysphonia 168. Because muscle weakness contributes to sleep‐disordered breathing, symptoms such as poor sleep, daytime somnolence, and morning headache are common, and often reflective of CO 2 retention 30, 91, 190…”

“…In these cases, prompt respiratory care with pulmonary bi-level positive airway pressure non-invasive ventilation-a common mode of respiratory support for patients with respiratory muscle weakness-can prolong life expectancy. 18 A possible limitation of this study was the fact that histopathological and genetic testing was only performed for a few patients. Further studies are recommended to confirm the histopathological and genetic findings of cases of paediatric RSS in Oman.…”
