Ventricular Fibrillation in the Wolff-Parkinson-White Syndrome

Abstract: To examine the risk of ventricular fibrillation in patients with the Wolff-Parkinson-White syndrome, we compared patients who had this syndrome and a history of ventricular fibrillation related to preexcitation with patients who had the syndrome without this history. Ventricular fibrillation occurred during atrial fibrillation, with rapid conduction over the accessory pathway, and these patients had a higher prevalence of both reciprocating tachycardia and atrial fibrillation (14 of 25 vs. 18 of 73, P = 0.004)… Show more

“…In the present case, the shortest R-R interval was 248 msec (Fig. 2D) and the preexitation was accelerated during exercise; these findings are in agreement with the criteria for high risk of the development of ventricular fibrillation and sudden death in patients with WPW syndrome (7,18,19). Moreover, this wide QRS tachycardia was refractory to antiarrhythmic drugs.…”

“…Patients in this category include those with Wolff-Parkinson-White syndrome who have rapid AF or syncope, those with certain arrhythmias resulting from congenital heart disease, and those with LV dysfunction who are at risk for, or who currently have, documented ventricular arrhythmias. While sudden arrhythmic death is an uncommon consequence of untreated Wolff-Parkinson-White syndrome, it is particularly devastating in these typically young patients with curable disease (15). Patients with congenital heart disease and certain high-risk arrhythmias may also have a preventable mortality risk with early intervention.…”

“…As taquicardias supraventriculares podem estar implicadas na morte súbita cardíaca tanto como fator causal direto como indireto, complicando uma doença cardíaca coexistente [16][17][18] . Em um levantamento de 290 casos de parada cardíaca abortada, publicado por Wang e cols.…”
