“…In patients with hemodynamic instability and higher surgical risk, TCC could serve as a less invasive management option although we found no difference in mortality between those who had early TCC as compared with early surgical repair (54 ± 32% vs 56 ± 23% respectively, P = 0.82). One of the most common factor associated with the poor outcomes of PIVSD closure is the presence of cardiogenic shock; however, if not treated with TCC or surgical repair, the mortality rate approaches 100%. Use of intra‐aortic balloon pump (IABP) can provide initial stabilization and could be used for hemodynamic support prior to closure of PIVSD either surgically or with TCC .…”

“…A delay in repair for 6 to 8 weeks results in an improved early mortality, but the cost is the death of a number of patients who might otherwise be salvaged by early operation. The current approach in most units, therefore, is to consider ventricular septal rupture as a surgical emergency 6 8 ) .…”

“…Postinfarction septal rupture has a poor prognosis when treated conservatively. Septal rupture after infarction should be considered for surgical repair unless a compelling reason exists to suggest otherwise 6 8 ) . Pseudoaneurysm of the left ventricle in even asymptomatic patients or small-sized aneurysms, tend to rupture 9 ) .…”
