Vestibular involvement in Myringitis Bullosa

Abstract: Not only is the cochlear part of the inner ear affected in patients suffering from MB, but the vestibular part as well. Sensorineural HL and vestibular abnormalities should both be considered as manifestations of MB.

“…The six studies involving patients with AOM included 70 subjects (age range 3.5-77) [39][40][41][42][43][44]. Four studies [39,40,43,44] comprising 50 patients with AOM demonstrated that 24 (48%) complained of dizziness or vertigo.…”

“…2. Due to the accompanying middle ear fluid, most of these patients would have conductive hearing loss, however, the sensorineural hearing would also be abnormal in more than half of the BM patients (Drendel et al, 2012; Hoffman and Shepsman, 1983; Hariri, 1990; Eliashar et al, 2004). The mechanism of reduced sensorineural thresholds in these patients is not clearly understood.…”

“…It can also demonstrate peaked curve with positive pressure, suggesting the acute nature of the fluid collected in the middle ear in BM (Kotikoski et al, 2003b). Apart from the hearing loss, the dysfunction of the entire vestibular system has also been objectively documented in the affected individuals of BM (Eliashar et al, 2004). Nevertheless, the hearing loss and the vestibular dysfunction are supposedly transient, and they recover after the relief of otitis media in most of these affected individuals (Eliashar et al, 2004).…”

“…Bullae involving the TM may also extend towards portions of the external auditory canal immediately adjacent to the TM. Objective cochlear and vestibular involvement manifested by sensorineural or mixed-type hearing loss and vertigo have been reported in patients suffering from MB (Eliashar et al 2004). Treatment includes analgetic and antibiotic therapy such as azithromycin which is efficacious against the major known pathogens causing MB.…”
