Vestibular Schwannoma: The Evolution of Hearing and Tumor Size in Natural Course and after Treatment by LINAC Stereotactic Radiosurgery

Abstract: Objective: To review the natural course of tumor size and hearing during conservative management of 151 patients with unilateral vestibular schwannoma (VS), and to evaluate the same parameters for the part of the group (n = 84) who were treated by LINAC stereotactic radiosurgery (SRS). Methods: In prospectively collected data, patients underwent MRI and complete audiovestibular tests at inclusion, during the conservative management period and after SRS. Hearing was graded according to the Gardner-Robertson (GR… Show more

“…The estimated TCP is consistent with clinical reports, in which prescription doses of at least 12 Gy for SRS and 50 Gy for IMRT yielded TCP values of >90%. 9 , 10 , 11 , 12 All NTCP predictions for SNHL were significantly higher for the clinical plans than for the 4π plans. The average NTCP was 10.0% higher for the clinical plans at 3 years and 18.4% higher at 5 years posttreatment.…”

“…All model parameters were selected on the basis of published clinical data on the relationship between treatment outcomes (tumor control and complication rates) and dose delivered to the tumor and cochleae. 9 , 10 , 11 , 12 Because the fractionation schemes varied widely among patients in this study, all plan doses were normalized to a reference dose of 2 Gy per fraction for radiobiologic modeling.…”

“…Spontaneous shrinkage of VS has been reported in 3–11% of VS managed conservatively [2–4,6,7,13], but few studies have characterized these tumors in detail with accurate imaging and volumetric analysis.…”

“…Most of the growing tumors grow slowly, frequently allowing a wait-and-scan strategy that usually consists of a first MRI 6 months after diagnosis, to detect the fast growing VSs that will require treatment, followed by an annual MRI [5]. Spontaneous shrinkage has rarely been observed [2–4,6,7].…”
