Video-assisted Thoracoscopic Surgery for Catamenial Hemoptysis: the Rationale of Preoperative Computed Tomography-guided Hook-wire Localization

Abstract: Catamenial hemoptysis is a rare disease. Hormone ablation therapy is the treatment of choice with multiple side effects. We report a case of pulmonary endometriosis with deep and changeable focus. Successful treatment was obtained with the combine use of computed tomography-guided hook-wire localization and video-assisted thoracoscopic surgery. Owing to benign and curable in nature, we suggest a more aggressive attitude toward this disease before proceeding to hormone ablation therapy. The role of preoperative… Show more

“…Preoperative CT-guided localization was suggested because of the intraparenchymal location of lesion, as seen on CT, and the lack of visible abnormalities found intraoperatively. 4 Several localization methods such methylene blue staining and hook wire techniques have been proposed, they have a high failure rate and risks of pneumothorax and intraparenchymal hemorrhage. A recently proposed intraoperative transthoracic ultrasonography could be another useful method in localizing lung lesion.…”

“…( Franceschini et al ) It has also been used to localize axillary sentinel lymph nodes or radiologically suspicious lymph nodes as part of the surgical management of breast cancer ( Caudle et al, 2015 ). The second most common reported use is the wire localization of small lung nodules to facilitate excision through a VATS procedures ( Park et al, 2017 , Chao et al, 2006 ).…”

“…There are different methods available to accurately locate nonpalpable lesions, including RSL, wire-guided localization (WGL), and radioguided occult lesion localization (ROLL), but their use for the treatment of endometriosis is rare. To our knowledge, only 2 studies using WGL in scar and pulmonary endometriosis [6,7] and 1 study using ROLL for the excision of an endometriotic lesion in the abdominal wall have been described [8].…”
