Virtual Guidance as a Tool to Obtain Diagnostic Ultrasound for Spaceflight and Remote Environments

Abstract: Diagnostically-adequate carotid and ophthalmic ultrasound examinations can be obtained by previously untrained operators with assistance from only an audio/video tutorial viewed in real time while scanning. This form of just-in-time training, which can be applied to other examinations, represents an opportunity to acquire important information for NASA flight surgeons and researchers when trained medical personnel are not available or when remote guidance is impractical.

“…This versatility comes with the limitation of needing an expert operator to obtain high-quality images and perform the measurements. Astronauts on the ISS have successfully performed the ocular US with remote guidance and have obtained good quality US images 26 . While MRI and US measurements of the same subject at the same time point showed excellent consistency 27 , it is not possible to use this data from different SANS ONSD studies interchangeably due to the variability in timing and measurement methods.…”

“…The subject also watched a medical video demonstrating a person performing self-ultrasound 15 . It was shown that both remote guidance and a computer training tool produce a higher quality of ultrasound images 6,9 . During the procedure, the subject performed the test while guided remotely by the sonographer, who was in a separate location from the subject.…”

“…4 Additionally, ultrasound examination of the eye (which allows for the rapid assessment of the cerebral edema) is easy enough to be taught and used by non-expert operators on the International Space Station (ISS). 5,6 To date, there is an increasing number of ultrasound teaching pilot studies. These kinds of activities, used as a supplement to gross anatomy courses, are beneficial even during the first years of study.…”
