Virus Load and Risk of Heterosexual Transmission of Human Immunodeficiency Virus and Hepatitis C Virus by Men with Hemophilia

Abstract: A high human immunodeficiency virus (HIV) load may increase the probability of HIV transmission by sexual contact, but the association of virus load of hepatitis C virus (HCV) with risk of HCV transmission is uncertain. HIV and HCV virus loads were examined in hemophilic men, as were risks of HIV and HCV transmission to their female partners in a hemophilia cohort in which most subjects are dually infected. A higher HIV load was associated with an increased risk of HIV transmission (odds ratio [OR], 1.31 per l… Show more

“…Seroprevalence rates varied from 2.0% to 4.8%. [20][21][22] Preliminary data from the HCV Partners Study, a cross-sectional study of heterosexual monogamous couples, provides the best available estimate of risk among U.S. couples. In this study, which excludes couples in which injection drug use is present in both partners, the prevalence of anti-HCV among 401 partners was 4.2%, with genotype concordance present in 2.7% of couples (Terrault N, unpublished data).…”

“…Third, intervention with HAART during the period of primary infection may significantly alter disease course and the spread of infection (34)(35)(36)(37). Finally, given that viral load appears to be related to transmission (38)(39)(40)(41), initiating therapy is likely to reduce transmission of the virus across various stages of illness because viral loads will be lower.…”

“…Although linking seropositive persons to care is an important goal in itself, increasing the proportion of infected individuals who are referred to and followup in care will also help to decrease rates of HIV transmission. Viral load is often decreased among persons who are linked to care and adhere to their treatment regimens (36,(77)(78)(79); lower viral load is associated with decreased rates of HIV transmission (38)(39)(40)(41).…”
