Cardiovascular autonomic neuropathy (CAN) is a severe complication of the advance stage of diabetes. More than 50% of diabetic patients diagnosed with peripheral neuropathy will have CAN, with clinical manifestations including tachycardia, severe orthostatic hypotension, syncope, and physical exercise intolerance. Since the prevalence of diabetes is increasing, a concomitant increase in CAN is expected and will reduce quality of life and increase mortality. Autonomic dysfunction is associated with reduced baroreflex sensitivity and impairment of sympathetic and parasympathetic modulation. Various autonomic function tests are used to diagnose CAN, a condition without adequate treatment. It is important to consider the control of glucose level and blood pressure as key factors for preventing CAN progression. However, altered biomarkers of inflammatory and endothelial function, increased purinergic receptor expression, and exacerbated oxidative stress lead to possible targets for the treatment of CAN. The present review describes the molecular alterations seen in CAN, diagnosis, and possible alternative treatments.

Painful diabetic neuropathy (PDN) is a serious and very common complication of diabetes mellitus (DM). It negatively affects the quality of life, increases morbidity and poses a financial burden on the health care system. Currently, treatment of PDN focuses on glycaemic control, while pathogenesis-oriented therapy has not yielded satisfactory results. The need to improve therapy remains. There is accumulating evidence on the potential benefit of nutritional interventions. The aim of this narrative review was to examine the potential benefit of dietary and nutritional supplementation for PDN management. According to preliminary research, supplementation with vitamin E, B-complex, omega 3 fatty acids, CoQ10 or N-acetylcysteine appears to yield promising results in improving PDN symptoms.

ZUSAMMENFASSUNGDie kardiale autonome diabetische Neuropathie (KADN) geht mit einer erhöhten Mortalität und Morbidität einher. Sie kommt auch in frühen Phasen der Entwicklung eines Diabetes mellitus vor. Die KADN ist unterdiagnostiziert, unter anderem weil die Screening-Tests zeitaufwändig und kostspielig sind, aber auch weil unsere Wahrnehmung hinsichtlich der Bedeutung der KADN unzureichend ausgeprägt ist. Dieser Artikel beschäftigt sich mit der Epidemiologie, der prognostischen Relevanz, Diagnostik, sowie Therapie der KADN.
