Numerous studies show the benefits of vitamin D: in the prevention of bone disease, supporting the immune system and the prevention and mitigation of cancer. Until recently, supplementation has been deemed unnecessary. Nonetheless, in the light of emerging evidence, some practitioners are adding vitamin D and its analogues (Anatomic Therapeutic Chemical (ATC) classification code A11CC) to their prescriptions. Therefore, the aim was to determine the medical insurance prescribing patterns of vitamin D in South Africa. Methods: A retrospective, cross-sectional drug utilisation study was conducted on a South African medical insurance administrator database for 2018. Products in ATC subgroup A11CC (vitamin D and analogues) were extracted and analysed. Results: A total of 302 patients received 1 164 prescriptions for a vitamin D or analogue product during 2018. The average age of patients was 47.14 (SD = 24.03) years, and the majority were female (58.28%). Very young patients and middle-aged patients received most of the prescriptions. Ergocalciferol (A11CC01) was the most frequently prescribed (59.79%), followed by alfacalcidol (A11CC), colecalciferol (A11CC05) and calcitriol (A11CC04). Ergocalciferol 50 000 IU tablets comprised the most frequently prescribed trade name product (59.45%), followed by alfacalcidol capsules (21.31%). Conclusion: Vitamin D was used across the age range. Vitamin D supplementation was primarily prescribed in the very young and older age groups, suggesting their need for additional supplementation. As the cost of additional supplements is relatively small compared with the treatment of deficiency disorders, vitamin D supplementation could have broad and positive effects on long-term health for very little outlay.

Determinants of Plasma 25-Hydroxyvitamin D Concentrations among Breast Cancer Survivors in Korea

We identified demographic, lifestyle, and clinical factors associated with vitamin D status among breast cancer survivors. The vitamin D prediction model may be a useful surrogate of circulating 25-hydroxvitamin D (25(OH)D) concentrations when this measure was not available. We included a total of 216 Korean breast cancer survivors aged 21–79 years who had been diagnosed with stage I to III primary breast cancer and had breast cancer surgery at least 6 months before enrolment. We used linear and logistic regressions to identify determinants for the plasma 25(OH)D concentrations and vitamin D insufficiency (plasma 25(OH)D concentration < 50 nmol/L). We observed that 48.85% of breast cancer survivors had a plasma 25(OH)D concentration less than 50 nmol/L. We identified the following determinants for plasma 25(OH)D concentrations: time since diagnosis (β = −0.005 for 1 month increment), supplementary vitamin D intake (β = 0.06 for 10 μg/day increment), season of the blood draw (β = 0.35 for summer; β = 0.32 for fall; β = 0.26 for winter vs. spring), smoking status (β = 0.28 for former vs. never), use of any supplement (β = −0.35 for non-use vs. use), and the parity number (β = −0.30 for three or more vs. one) were associated with the plasma 25(OH)D concentrations. In addition to the aforementioned variables, body mass index (BMI) was associated with the prevalence of vitamin D insufficiency. We identified the determinants for the plasma 25(OH)D concentrations among Korean breast cancer survivors. Future studies are needed to investigate the role of vitamin D in the progression of breast cancer among Korean breast cancer survivors.

Objectives In this study, the aim is to provide a more detailed understanding of vitamin D metabolism by evaluating the correlation between 1,25-dihydroxyvitamin D (1,25(OH)2D) and 25-hydroxyvitamin D (25(OH)D) according to the variations in measurement methods and clinical conditions. Methods We searched PubMed, Embase, and Web of Science for studies reporting correlation results between 1,25(OH)2D and 25(OH)D. We performed a meta-analysis based on the correlation results of 1,25(OH)2D and 25(OH)D in different clinical conditions. We included a total of 63 studies and our laboratory’s results in the meta-analysis. The studies were categorized into high-quality methods group (HQMG), medium-quality methods group (MQMG), and low-quality methods group (LQMG) based on the 25(OH)D and 1,25(OH)2D measurement. Results In the healthy, renal disease, and other disease groups, the highest correlation values were observed in the studies categorized as HQMG, with values of 0.35 (95 % CI; 0.23–0.48), 0.36 (95 % CI; 0.26–0.42), and 0.36 (95 % CI; 0.22–0.48), respectively. Significant statistical heterogeneity was observed in the healthy, renal disease, and other disease groups, with I2 values of 92.4 , 82.7, and 90.7 %, respectively (p<0.001). Both Funnel plots and the results of Egger’s and Begg’s tests indicated no statistically significant bias across all studies. Conclusions A significantly low correlation was found between 25(OH)D and 1,25(OH)2D. However, higher correlations were found in the studies categorized as HQMG. Various factors, including methodological inadequacies and disparities, might contribute to this. In the future, with more accurate and reproducible measurements of 1,25(OH)2D, a clearer understanding of vitamin D metabolism will be achieved.
