Vitrectomy with a modified temporal inverted limiting membrane flap to reconstruct the foveolar architecture for macular hole retinal detachment in highly myopic eyes

Abstract: A vitrectomy and modified C-shaped inverted temporal ILM flap is effective for closing MHs, reattaching the retina, restoring the foveolar architecture and significantly improving the postoperative BCVA in MHRD patients. This technique is feasible, and we propose 'presumed' Müller cell cone repair in MHRD surgery.

“…We stress again that further studies are needed to make more definitive conclusions on this issue. Indeed, the advantage of I-ILM flap technic had been reported, while there are some alternative methods that have been proposed (Michalewska et al 2010(Michalewska et al , 2015Morizane et al 2014;Gekka et al 2015;Chen & Yang 2017;Ho et al 2017;Leisser et al 2017;Ozdek et al 2017;Yuan et al 2017).…”

“…After Michalewska et al's first report, there have been many studies on the inverted ILM flap technique with or without some modification (Grewal & Mahmoud 2016;Yuan et al 2017). These techniques include temporal inverted ILM flap by Michalewska et al (2015), Leisser et al (2017), Ho et al (2017), pedicle ILM flap by Gekka et al (2015), autologous and pedicle ILM flap with perfluoro-n-octane by Ozdek et al (2017), autologous ILM flap by Morizane et al (2014) and double internal limiting membrane insertion by Chen & Yang (2017).…”

“…The ideal modality for treating MHRD remains unknown. Recently, MHRD surgical intervention by pars plana vitrectomy (PPV) with an inverted internal limiting membrane (ILM) flap [2] and its modified techniques have been developed to improve retinal attachment and macular hole (MH) closure rate [3][4][5][6].…”

“…Several methods have been proposed to increase the MH closure rate after vitrectomy with internal limiting membrane (ILM) peeling, such as inverted epiretinal ILM flap [3], inverted ILM flap insertion [4] with or without blood coverage of the hole [5], temporal, or superior single-layer inverted ILM flap [6, 7]. Making an epiretinal flap to cover the hole or creating ILM flaps around the MH to insert into the hole may be technically demanding, and ILM flaps with an appropriate size and shape cannot always be created in a staphylomatous eye with epiretinal membrane and RD.…”
