VP55.01: Pregnancy with an amniocele, conservatively managed until fetal maturity: a case report

Abstract

Virtual poster abstracts labour and perinatal and delivery outcomes in a cohort of uncomplicated singleton term pregnancies. Methods: Prospective multicentre observational study conducted at four tertiary centres between January 2016 and October 2019. Low-risk term pregnancies with spontaneous onset of labour were included. The Doppler measurements were obtained in between uterine contractions in women admitted for early labour as the CPUR was computed as the cerebroplacental ratio converted into MoMs divided by the mean uterine artery (UtA) PI MoM. Doctors and midwives involved in the clinical management of the women were blinded to the results of the Doppler evaluation. Mode of delivery and perinatal outcome were compared between pregnancies with reduced CPUR MoM, defined as CPUR within the lowest decile of the study population, and those with normal CPUR. Results: Overall, 804 women were included. The rate of obstetric intervention for suspected fetal distress in labour was almost six-fold higher among cases with CPUR within the lowest decile compared to those with CPUR above the 10th percentile (21/75, 28%, vs 33/729, 4.5%; p < 0.001). Furthermore, a higher rate of composite adverse perinatal outcome was found in fetuses with CPUR <10th percentile compared to those with CPUR ≥10th percentile (10/75, 13.3%, vs 22/729, 3%; p < 0.001). CPUR within the lowest decile showed low sensitivity and positive predictive value for obstetric intervention (38.9% and 28.0%, respectively) and for composite adverse perinatal outcome (31.3% and 13.3%, respectively). Conclusions: Data on a wide cohort of low-risk term pregnancies in early labour showed an association between CPUR within the lowest decile with an higher incidence of obstetric intervention due to fetal distress and composite adverse perinatal outcome.
