Warty dyskeratoma/focal acantholytic dyskeratosis – an update on a rare oral lesion

Abstract: Warty dyskeratoma/FAD are uncommon oral lesions which are not encountered in the daily practice of oral pathologists. The absence of an association of oral lesions with the pilosebaceous apparatus suggests that they are probably distinctly different from cutaneous ones. As such, we suggest the histologic term isolated FAD for oral lesions, rather than WD.

“…Allon and Buchner 1 published a comprehensive review of oral WD, in which they catalogue 41 total cases, including a case of their own occurring on the buccal mucosa of an 81-year-old woman. Since their publication in 2012, only 1 additional case of oral WD has been reported (excluding the current case); this was by Steele et al 3 in 2014 who described a case of WD occurring on the buccal mucosa of a 60-year-old white man.…”

“…Warty dyskeratoma (WD), also known as isolated Darier disease or focal acantholytic dyskeratosis , 1 is a rare mucocutaneous lesion. It usually presents as a solitary, asymptomatic umbilicated papule, most often in the head and neck region of middle- to older-age adults.…”

“…It usually appears as an asymptomatic solitary reddish-brown papule or nodule, with a keratotic umbilicate centre, located in the head and neck of young adults. Multiple lesions and several cases located on oral mucosa have been reported [2, 3]. The histopathology shows a characteristic pattern of acantholytic dyskeratosis that is not pathognomonic.…”

“…Typically it is a distinctive histological feature of Darier's disease, transient focal acantholytic dyskeratosis or Grover disease, and Hailey-Hailey disease [3]. Nevertheless FAD has been observed as an incidental finding in several skin lesions such as compound and junctional nevi, scars, seborrheic keratosis, basal cell carcinoma, and squamous cell carcinoma [5].…”
