Water Jet Assisted Laparoscopic Partial Nephrectomy Without Hilar Clamping in the Calf Model

Abstract: In this more stringent and robust survival calf model Hydro-jet assisted LPN can be performed without hilar vessel control, thus, completely avoiding warm ischemia. This approach has the potential to decrease the level of technical difficulty inherent in LPN.

“…Moinzadeh et al [15] performed LPN on 10 calves using the Helix Hydro-jet (Erbe Elektromedizia GmbH, Turbingen, Germany) at a pressure setting at 450 psi with BIClamp (Erbe Elektromedizia GmbH, Turbingen, Germany) and were able to perform on 18 of 20 kidneys without hilar control, with an operative time of 173 min and blood loss of 60 ml. Others have also used this technology in the porcine model with and without vascular control [16,17] with the use of metallic clips, or vascular clips and an endoscopic gastrointestinal stapler to secure the centrally located blood vessels and collecting system.…”

“…Since then, waterjet cutting has become an established technique in different surgical fields [2]. The technique is used clinically for cutting soft tissues like liver tissue [3][4][5][6] and experimentally for dissecting spleen tissue [7,8], kidney tissue [9][10][11][12] and brain tissue [2,13,14]. Waterjets have also been investigated for cutting hard materials such as bone and bone cement [15][16][17][18].…”
