Wedge Resection for High Astigmatism After Penetrating Keratoplasty for Keratoconus: Refractive and Histopathologic Changes

Abstract: Wedge resection is a safe and moderately effective procedure in the correction of high astigmatism after penetrating keratoplasty for keratoconus. Histopathologic changes confirm a true late progression of the disease in the host cornea. Keratoconus may be a disease that affects the entire cornea, and surgical resection does not cure the disease.

“…Nevertheless, low predictability is a major concern with this procedure. [73][74][75][76][77] The manual technique of wedge resection is difficult and requires expert skill. In particular, the second incision is more difficult because the cornea is more flaccid as a result of the first incision.…”

“…Wedge resection is an option for high postkeratoplasty astigmatism but is limited by low predictability and stability. 20 Laser refractive surgery with photorefractive keratectomy or laser in situ keratomileusis has been reported 6-9 with good predictability but may be limited by the amount of the spherical error and astigmatism that can be corrected, posttreatment haze, and concerns about reepithelialization in patients with a poor ocular surface.…”

“…Subsequently, there have been several reports describing the application of this surgical technique for treating keratoconus recurrence after PKP. 9,10 Corneal wedge resection represents an effective and reasonably predictable method. Nevertheless, similar to other…”
