Wernicke–Korsakoff syndrome associated with mtDNA disease

Abstract: Introduction: Wernicke encephalopathy (WE) and Wernicke–Korsakoff syndrome (WKS) are well-known disorders caused by thiamine deficiency. In addition to the classical concept of these diseases, some literature data suggest a connection between mitochondrial dysfunction and WE/WKS. Psychotic disorders and WKS seem to run in families, as the deficiency of the oxidative phosphorylation can be a trigger factor in psychotic events and WE/WKS as well. We present a patient harbouring the m.A3243G mtDNA mutation with t… Show more

“…( 43 ). The third patient ( case 22 ) had Wernicke-Korsakoff syndrome with psychotic, cognitive, and negative symptoms as well as psychomotor retardation despite aripiprazole treatment ( 46 ). After switching to cariprazine 3.0 mg/day, improvements in both psychotic and negative symptoms were detected without any side-effects, which was followed by cognitive improvement 3 months later ( 46 ).…”

“…Furthermore, there are post-hoc analyses demonstrating improvement of cognitive dysfunction in different psychiatric disorders after CAR treatment (29)(30)(31)(32). indications: mitochondrial encephalomyopathy and lactic acidosis (MELAS syndrome) due to the mutation m.A3243G where the predominantly negative symptoms and cognitive dysfunction improved (33); substance use disorder (e.g., cocaine, alcohol, methamphetamine) (34,35); obsessive-compulsive disorder as add-on therapy (36) and borderline personality disorder (37). Our study provides data for cariprazine's efficacy in non-motor symptoms of HD especially in loss of motivation, apathy, anhedonia, depression and cognitive symptoms.…”

“…É causada pelo uso crônico de álcool, deficiência de vitamina B1 (tiamina) e em alguns casos são encontradas alterações genéticas no DNA mitocondrial. Testes com Cariprazina (antipsicótico) e suplementos vitamínicos mostram que ambos podem ser eficazes no tratamento da doença (Jimoh, 2020).…”
