Wertigkeit der quantitativenß₂-Mikroglobulin- und Paraprotein-Bestimmung im Serum von Patienten mit multiplem Myelom und Waldenströms Makroglobulinämie

Abstract

Serumß 2 -microglobulin andserum paraproteins (SPPR) were determinedin 132patients with IgG myeloma, 33 patients with Ig A myeloma and. 38 patients with Waldenström's macroglobulinaemia (WM) using a competitive enzyme-immunoassay and a fully automated serum electrophoresis System connected on line via RS-232C to a microprocessor-based terminal which displayed the electropherogram. The measured ßi-microglobulin concentrations were significantly (p< 0,0005) increased for patients with IgG-or IgA-myeloma and WM compared to the control group of healthy adults. No correlation was found between serum ß 2 -microglobulin and SPPR for the respective patient groups. For 40 patients with liver cirrhosis and 10patients with chronicpyelonephritis significantly (p < 0.025, p < 0.005) increasedamounts of ß 2 -microglobulin were found respectively. The results suggest, that ß 2 -microglobulin is a useful tool in order to evaluate the kidney manifestation of multiple myeloma or WM. The likewise increased amounts of ß 2 -microglobulin in chronic inflammatory diseases e.g. liver cirrhosis and chronic pyelonephritis limit ß 2 -microglobulin äs an adequate marker for lymphoproliferative disorders. Neben den beiden Parametern produzieren die Harnwege (5-7). Bei ca. 50% der Patienten stellen die Tumorzellen einen Piasmozytom-Faktor, der auf Monozyinfektiösen Komplikationen die Todesursache dar (8).
