Background: Parotid tumours (PTs) have a variety of pathological types, and the surgical procedures differ depending on the tumour type. However, accurate diagnosis of PTs from the current preoperative examinations is unsatisfactory. Methods: This retrospective study was approved by the Ethics Committee of our hospital, and the requirement for informed consent was waived. A total of 73 patients with PTs, including 55 benign and 18 malignant tumours confirmed by surgical pathology, were enrolled. All patients underwent diffusion-weighted imaging (DWI), dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI), susceptibility-weighted imaging (SWI), T2weighted imaging (T2WI), and T1-weighted imaging (T1WI). The signal uniformity and capsule on T2WI, apparent diffusion coefficient (ADC) derived from DWI, semi-quantitative parameter time-intensity curve (TIC) pattern, and quantitative parameters including transfer constant (K trans ), extravascular extracellular volume fraction (V e ), wash-out constant (K ep ) calculated from DCE-MRI, and intratumoural susceptibility signal (ITSS) obtained from SWI were assessed and compared between benign and malignant PTs. Logistic regression analysis was used to select the predictive parameters for the classification of benign and malignant parotid gland tumours, and receiver operating characteristic (ROC) curve analysis was used to evaluate their diagnostic performance. Results: Malignant PTs tended to exhibit a type C TIC pattern, whereas benign tumours tended to be type A and B (p < 0.001). Benign PTs had less ITSS than malignant tumours (p < 0.001). Multivariate analyses showed that ADC, V e , and ITSS were predictors of tumour classification. ROC analysis showed that the area under the curve (AUC) of ADC, V e , ITSS, and ADC combined with V e were 0.623, 0.615, 0.826, and 0.782, respectively, in differentiating between malignant and benign PTs. When ITSS was added, the AUCs of ADC, V e , and ADC combined with V e increased to 0.882, 0.848, and 0.930, respectively. Diagnostic Accuracy of Fine‐Needle Biopsy for Salivary Gland Neoplasms in a Community Otolaryngology Practice

Objective To assess the diagnostic accuracy of fine-needle biopsy (FNB) of salivary gland neoplasms via ultrasound (US) or palpation guidance by an otolaryngologist in a community practice. Study Design Retrospective chart review. Setting Community otolaryngology practice. Methods Retrospective analysis was conducted for all office-based salivary gland FNBs from a community practice from 2005 through 2018. There were 433 FNBs performed among 370 patients. The likelihood of achieving a diagnostic result based on method (US vs palpation guidance) was calculated. Of this cohort, 196 cases had surgical follow-up (parotid gland, n = 168; submandibular gland, n = 28). Correlation of preoperative FNB results to final surgical pathology was performed and measures of diagnostic accuracy computed. Results US-guided FNBs were more likely to achieve a diagnostic result than FNBs obtained via palpation guidance ( P = .00002). Parotid gland FNBs demonstrated a sensitivity and specificity of 78.57% and 92.44%, respectively. Submandibular FNBs demonstrated a sensitivity and specificity of 57.14% and 93.74%. Conclusion FNBs performed under US guidance are more likely to achieve a diagnostic specimen than those performed under palpation guidance. FNBs of parotid gland tumors may be assessed with diagnostic accuracy in the community setting that is similar to that achieved at tertiary care centers. Further research is needed to ascertain whether this finding extends to FNBs of submandibular gland tumors. Offering this procedure at point of care in the community may improve compliance and reduce wait time.

ZusammenfassungDie Behandlung von Speicheldrüsenerkrankungen stellt ein wichtiges Segment der HNO-Heilkunde dar. Von der Vielzahl von Speicheldrüsenerkrankungen sieht der einzelne HNO-Arzt allerdings möglicherweise nur wenige Fälle pro Jahr und chirurgische oder endoskopisch minimal-invasive Therapiekonzepte spielen eine wichtige Rolle. Daher kann der Erkenntnisgewinn nicht nur von prospektiven klinischen Studien getragen werden, sondern heute im Wesentlichen durch Meta-Analysen und potentiell auch von Registerdaten. Während eine Vielzahl guter Indikatoren vorhanden ist, um die Funktion der erkrankten Speicheldrüse und deren Besserung unter Therapie zu messen, sind Patient-reported outcome measures (PROMs) bislang nicht ausreichend entwickelt worden. Es ist zu fordern, dass diese Indikatoren auch konsequent in klinischen Studien eingesetzt werden. Perspektivisch könnten dieselben Indikatoren auch für Qualitätskontrollen in der ambulanten und stationären Routine angewandt werden. Die Rahmenbedingungen für einen qualitativ hochwertigen Wissenserwerb sind durch die HNO-Facharzt-Ausbildung, die Verpflichtung der Weiterbildung und durch zertifizierte Speicheldrüsen-Kurse prinzipiell sehr gut, die Vorgaben für spezifische Qualitätsstandards für die Behandlung von Patienten sind jedoch wenig ausgearbeitet. In Abgrenzung zu anderen Fachgebieten, die sich mit Speicheldrüsenerkrankungen beschäftigen, sollte die HNO-Heilkunde Standards mit hoher Qualitätsforderung für die Behandlung von Patienten mit Speicheldrüsenerkrankungen entwickeln.
