What work has to be done to implement collaborative care for depression? Process evaluation of a trial utilizing the Normalization Process Model

Abstract: BackgroundThere is a considerable evidence base for 'collaborative care' as a method to improve quality of care for depression, but an acknowledged gap between efficacy and implementation. This study utilises the Normalisation Process Model (NPM) to inform the process of implementation of collaborative care in both a future full-scale trial, and the wider health economy.MethodsApplication of the NPM to qualitative data collected in both focus groups and one-to-one interviews before and after an exploratory ran… Show more

“…In the study by Sanchez et al [35], the use of a physician champion (along with early “identification and treatment of the most problematic patients”) was key to helping the primary care team understand the model. In the same vein, other studies [29, 40, 42] found it important to clarify roles and responsibilities between the participants from primary and secondary care.…”

“…Nine studies aimed to identify barriers and/or facilitators to the implementation of collaborative care [9, 30, 32, 34, 36–40]. Some studies stated their aim more broadly in terms of exploring the implementation [6, 31] or the sustainability of collaborative care [41], how change occurred [33], or the work needed to integrate collaborative care in practice [42]. …”

“…Following the thematic analysis we conducted a further theory-driven analysis of the data guided by the four main constructs of normalisation process theory 24 (coherence, cognitive participation, collective action and reflexive monitoring), detailed in Box 1, building on a previous process evaluation in which we had used the normalisation process model 71 to identify the work required to implement collaborative care for depression. 72 Our analyses aimed to identify barriers to and facilitators of the successful implementation of collaborative care into UK primary care.…”

“…196 In this chapter we report the evaluation that explored the processes at work when people and groups encounter, and seek to embed, a new set of practices or ways of doing things. 191,197 We used process data collected during the trial and also asked a sample of those involved in the study to reflect, with the benefit of hindsight, on the processes of the trial. In addition, during stakeholder consultations, we asked those who had not been engaged in the intervention, or in the trial, to think about the findings of the study and how these might relate to their own current practice or service.…”
