Which treatment for large rectal adenoma? Preoperative assessment and therapeutic strategy

Abstract: In the present review the authors discuss the standard ways of preoperative work-up for a suspected large rectal non-invasive lesion, comparing East and West different attitudes both in staging and treatment. Looking at the literature and analyzing recent personal data, neither pit-pattern classification, nor EUS, nor biopsy histology, nor lifting sign verification, nor digital examination allow a specificity of more than three fourth of such cases. The authors disquisition about which optimal treatment exclud… Show more

“…The most important advantage in using TEM, we think, is the fact that visualization of the fistula is excellent without any incision in healthy tissue such as in York-Mason procedure or perineal approach. As described in the past literatures [3,4,12,13] and the authors know from our experiences, TEM has a lot of technical advantages compared with conventional transanal approach. The magnified threedimensional image obtained via the binocular endoscope enables surgeons to identify, for example, tiny tissue structures, difference between healthy tissues and ischemic tissues, and so on.…”

“…More than 30 years ago, TEM revolutionized the technique and outcomes of transanal surgery, its indication rapidly extending from the treatment of large rectal adenomas to early rectal cancer. More recently, its potential role in combination with neoadjuvant therapies for the treatment of more invasive cancer has been discussed.…”

“…1984 m. G. Buess pristatė transanalines endoskopines operacijas (TEM/TEO) [6].

“…Atliekant viso sienos storio eksciziją, pašalinimas vienu bloku labiau tikėtinas, tačiau suartinti ir susiūti rezekcijos kraštus nenaudojant tempimo ne visada įmanoma, be to, didėja pilvaplėvės perforacijos rizika. Nors tai nėra labai didelė komplikacija [8,10], tačiau perforacija turi būti pastebėta ir sandariai užsiūta. TEO cirkuliari sandari siūlė irgi techniškai sudėtinga, reikalaujanti didelės patirties [7].…”
