Who are excellent lithium responders and why do they matter?

Abstract: DSM and ICD operational criteria. According to the DSM-5, the core features of depression are depressed mood and anhedonia. ICD-10 adds a third core item, "decreased energy or increased fatigue". These core features have been identified clinically to be central to depression and are included in the six-item version of the Hamilton Depression Rating Scale, along with guilt feelings, psychic anxiety and psychomotor retardation 7

“…This classic episodic pattern of illness involving r ecurrent, r ecognisable mood episodes separated by periods of complete symptom-free r emission indicates the likelihood of a good response to lithium. In addition, lithium is likely to be more effective in severe mania, and in patients with a family history of a classic bipolar disorder (Alda, 2017) and in the absence of comorbidity. For guidance regarding how to prescribe and monitor lithium, see The Lithiumeter, (Malhi et al, 2016b).…”

“…Onethird of lithium-treated patients shows a pattern of complete clinical response, with absence of mood recurrences and, importantly, return to normal levels of functioning as well as to good quality of life. 31,32 Thus, lithium modifies substantially the trajectory of illness in BD patients, a characteristic that makes this drug a unique therapeutic tool in the field of psychiatry. The identification of reliable clinical predictors of lithium response has therefore gained attention.…”

“…Additionally, it is thought that there are certain subpopulations or ‘pheno-biotypes’ within bipolar disorder that may respond better to certain treatments 98. For example, it is well known that within bipolar disorder, patients may be categorised as ‘lithium responders’ or ‘non-lithium responders’ 99. While a review is beyond the scope of this article, an increasing number of groups are researching personalised medicine approaches to predict which treatments patients will respond best to.…”
