Whole blood genome-wide transcriptome profiling and metagenomics next-generation sequencing in young infants with suspected sepsis in a low-and middle-income country: A study protocol

Abstract: Conducting collaborative and comprehensive epidemiological research on neonatal sepsis in low- and middle-income countries (LMICs) is challenging due to a lack of diagnostic tests. This prospective study protocol aims to obtain epidemiological data on bacterial sepsis in newborns and young infants at Kamuzu Central Hospital in Lilongwe, Malawi. The main goal is to determine if the use of whole blood transcriptome host immune response signatures can help in the identification of infants who have sepsis of bacte… Show more

“…To date, this pioneering research has found numerous instances of expected human rhinoviruses, enteroviruses, influenza A virus, coronavirus OC43, and respiratory syncytial virus, rotavirus, Torque-teno virus, human papillomavirus, human beta herpesvirus 7, along with novel cyclovirus, vientovirus, gemycircularvirus, and statovirus (17)(18)(19)(20)(21)(22)(23). The blood DNA virome in several thousand asymptomatic humans has been reported by researchers in the USA and Singapore (Human Longevity Ltd.) 94 different viruses including DNA viruses, parvovirus and RNA viruses were identified in 42% of patients studied (34). RNA viruses included herpesviruses, anelloviruses, papillomaviruses, polyomaviruses, adenovirus, HIV, HTLV, hepatitis B and C, parvovirus B19 and influenza.…”

“…Study design, setting & population: This study was a pragmatic cohort study, and secondary analysis of data from a parent study that aimed to identify molecular signatures of bacterial sepsis in young infants with suspected sepsis (12). In the parent study, infants below 3 months of age were sequentially enrolled "around the clock" at the time they presented with suspected sepsis before, to the neonatal or pediatric units at Kamuzu Central Hospital in Lilongwe, Malawi between June 5 th , 2018, and April 6 th , 2020.…”

“…All infants also had a standardized aerobic blood culture (minimum 2 mL of blood) collected within 4 hours of presentation, using a rigorously implemented aseptic protocol (with in-training of all front-line healthcare providers prior to study initiation), as detailed (13). Infants also had cerebrospinal fluid (CSF) and urine cultures collected as clinically indicated.…”
