Whole Genome Sequencing of Carbapenem-Resistant Klebsiella Pneumoniae : Evolutionary Analysis for Outbreak Investigation

Abstract: Aim: Carbapenemase-resistant Enterobacteriaceae represents a major concern in hospital setting. Materials & methods: The evolutionary history of carbapenem-resistant Klebsiella pneumonia strains was analyzed by core genome multilocus sequence typing and Bayesian phylogenesis by whole genomes sequencing. Results: A great increase carbapenem-resistant K. pneumoniae causing blood stream infection was observed in the years 2015–2016. At multilocus sequence typing (MLST), they were prevalently ST512 and ST101. … Show more

“…Many studies of CPE outbreak investigations have focused on clonal transmission, relying upon single-nucleotide polymorphism distances indicative of clonal bacterial spread to determine genomically-linked transmission 10 , 18 , 19 . As plasmid-mediated transmission accounts for a significant prevalence of CPE transmission at a population-level, plasmid identification and linkage would be necessary for accurate CPE outbreak investigation 20 .…”

“…The spread of CR-KP has gradually increased and even led to outbreaks (Kizilay et al, 2020;Pellicé et al, 2021). Molecular epidemiology plays an important role in tracking the spread of MDR bacteria to provide data for controlling their dissemination (Fontana et al, 2020). Methods for studying the molecular epidemiology of CR-KP include multilocus sequence typing (MLST), pulsed-field gel electrophoresis, enterobacterial repetitive intergenic consensus-polymerase chain reaction, and metagenomics next-generation sequencing.…”

“…Multilocus sequence typing (MLST) and acquired antimicrobial resistance genes were identified by using the online CGE tool ( https://cge.cbs.dtu.dk ). Phylogenetic relationships were evaluated by core genome multilocus sequence typing (cgMLST) analysis, followed by generation of a minimum-spanning tree by using SeqSphere+ software (Ridom GmbH, Muenster, Germany) [ 29 ]. Virulence genes were predicted by using the online tools VFanalyzer ( http://www.mgc.ac.cn/cgi-bin/VFs/v5/main.cgi?func=VFanalyzer ) and Kaptive Web ( http://kaptive.holtlab.net/ ).…”
