Wide Combined Thin Free SCIA/SIEA Flap

Abstract: A thin skin flap is often required for optimal resurfacing of particular areas of the body such as oral lining, hands, and feet. Using wide and thin flaps provides a higher level of esthetic and functional results in reconstructive surgery. Recently, the authors also applied the thinning procedure for free SCIA/SIEA flaps. From 2003 to 2006, 11 patients underwent soft tissue reconstruction using thin free SCIA/SIEA flaps. In this procedure, deep subcutaneous fat tissue was removed totally while superficial sub… Show more

“…Although the color and texture of the groin skin mismatch that of the facial skin, the soft tissue in this area is more malleable and pliable than that of the thigh area. Thus, groin flap is a potentially excellent option for facial volume restoration instead of skin defect repair 14,15 . The groin vessel system is considered a relative constant vascular pattern, consisting of both SIEA and SCIA branches.…”

“…Thus, groin flap is a potentially excellent option for facial volume restoration instead of skin defect repair. 14,15 The groin vessel system is considered a relative constant vascular pattern, consisting of both SIEA and SCIA branches. These 2 vascular branches interrelate with each other, and anatomic variety does exist, which precluded the groin flap from a workhorse flap in facial reconstruciton.…”

“…The idea of the SCIA/SIEA combined flap to harvest thin free flap for soft tissue reconstruction was first reported by Nasr (Nasr & Aydn, 2008). They included both the SIEA and SCIA pedicle when the common trunk origin of these two vessels was found to have a larger diameter of the pedicle artery for a safe micro‐vessel anastomosis.…”

“…Nasr et al used a thin SIEA free flap for resurfacing areas of tissue loss on the hands. They removed the deep subcutaneous fat totally while superficial subcutaneous tissue was thinned and a fat tissue layer was left under the skin to protect the subdermal network [4].…”
