Background/Aim: Urban Self-Employed Social Health Insurance (USSHIP), which is similar to Community-based health insurance schemes CBHI, USSHIP, is a package in the National Health Insurance Scheme NHIS specific but is not limited to self-employed individuals in Nigeria which has been rarely explored as instrument in financing healthcare in Nigeria. This study was aimed to assess the Willingness-to-Pay (WTP) for USSHIP and its determinants amongst the self-employed in Port Harcourt. Methodology: This was a cross-sectional descriptive study of 204 self-employed individuals selected from various trade association in Port Harcourt using semi-structured interviewer-administered pre-tested questionnaires. An iterative bidding approach of the contingent valuation method was used to elicit maximum WTP for the schemes. A multivariate logistic regression analysis was done to determine predictors to WTP. Data collected were analysed using the Statistical Package for Social Science (SPSS), version 23 software. Results: The results show that 89.7% were willing to enrol into the program; the median WTP for the scheme per person/per month is ₦300 ($0.83). The predictors of WTP were marital status, level of education and mode of payment of healthcare. Conclusion: Sequel to the findings of this study, the WTP amount for social health insurance program by the self-employed is too small, the implications for these findings imply that with the amount stated, the program cannot be successfully implemented. Recognizing this low amount stated for WTP for USSHIP, which is insufficient to scale up the scheme, the government support for the program is highly recommended.

Background Nepal's Interim Constitution of 2007 addresses health as a fundamental right, stating that every citizen has the right to basic health services free of cost. Government of Nepal formed a Social Health Security Development Committee as a legal framework to start implementing a social health security scheme after the National Health Insurance Policy came out in 2013. The program has aimed to increase the access of health services to the poor and the marginalized, and people in hard to reach areas of the country, though challenges remain with financing. Several aspects should be considered in design, learning from earlier community-based health insurance schemes that suffered from low enrollment and retention of members as well as from a pro-rich bias.Method A community based cross-sectional comparative study was conducted in Baglung district of Nepal to find out and compare the health service utilization and direct out-of-pocket health care expenditure among the 225 insured and 225 uninsured households under the national health insurance program of Nepal. Insured households were randomly chosen from study area and uninsured households were selected by using neighborhood method.Result The study focused that insurance status was strongly associated with heath service utilization with odds ratio 1.774 (95% CI = 1.127-2.791, P = 0.013). The study also depicts that insurance status was major determinants of out of pocket health expenditure. The median out-of-pocket health expenditure among insured households Nepalese Rupees 200 while among uninsured was 1225, which was statistically significant (p = <0.001 MW-U test). Among uninsured group, non-dalit were 2.846 times more likely to utilize health services compared to the dalit (p = 0.003) but it was not significant (p = 0.47) among insured group.Conclusion Nepal’s Health Insurance Board is responsible for purchasing the quality health care service and make available at possible nearest point of its member. Our study reveals that Occupation, perceived health status, wealth status played significant role with health service utilization among uninsured group while it was not significant among insured group. Among both insured and uninsured group none of the study variables were found significant with out-of-pocket health expenditure.

Willingness to Enroll in Social Health Insurance and associated factors among Household Heads in Obio/Akpor Local Government Area of Rivers State

Background: Social health insurance (SHI) schemes cover the healthcare needs of the informal sector workers, provide them with financial protection, minimize the equity gap, reduce Out-Of-Pocket (OOP) spending, and enhance utilization of the healthcare system. Objective: To determine the willingness to enrol in SHI and its associated factors among household heads in Obio-Akpor Local Government Area (LGA) of Rivers state. Method: This community-based cross-sectional study was carried out using the clusters and the EPI random walk sampling technique to obtain data from 205 selected household heads who were administered with an interviewer-administered questionnaire. The collected data was analysed using SPSS version 21. Result: According to the findings, majority of the respondents were males (58.5%), aged between 31 to 45 years (52.7%), married (57.1%), completed the tertiary level of education (65.4%), employed or self-employed (95.1%), from a family size of 3 to 4 (56.1%) and earned 50,000 to 100,000 Naira monthly (27.8%). Also, 63.0% are not enrolled in any health insurance scheme out of which 60.5% are not willing to enrol, with lack of trust in the management of the scheme, no believe in paying for sickness or have other means of meeting their healthcare needs identified as the major reasons. However, younger age, higher educational level, employment status, and earning >50,000 to 100,000 monthly had statistically (p<0.05) significant effect on the willingness of the respondents to enrol on SHI Conclusion: This result showed a low level of enrolment in health insurance as well as willingness to enrol in Shi, hence, both insurance companies, government and even Non-Governmental Organisation should carry out strategic campaigns to dispel false rumours about SHI while outlining the merits.
