Rationale: Although asthma is usually considered to originate in childhood, adult-onset disease is being increasingly reported.Objectives: To contrast the proportion and natural history of adultonset versus pediatric-onset asthma in a community-based cohort. We hypothesized that asthma in women is predominantly of adult onset rather than of pediatric onset.Methods: This study used data from the Coronary Artery Risk Development in Young Adults (CARDIA) cohort in the United States over a 25-year period. Adult-and pediatric-onset asthma phenotypes were studied, as defined by age at onset of 18 years or older. Subjects with asthma were categorized by sex, obesity, atopy, smoking, and race by mean age/examination year, using a three-way analysis of covariance model. Natural history of disease was examined using probabilities derived from a Markov chain model. Measurements and Main Results:Asthma of adult onset became the dominant (i.e., exceeded 50%) phenotype in women by age 40 years. The age by which adult-onset asthma became the dominant phenotype was further lowered for obese, nonatopic, eversmoking, or white women. The prevalence trend with increasing time for adult-onset disease was greater among subjects with nonatopic than atopic asthma among both sexes. Furthermore, adult-onset asthma had remarkable sex-related differences in risk factors. In both sexes, the quiescent state for adult-onset asthma was less frequent and also "less stable" over time than for pediatric-onset asthma.Conclusions: Using a large national cohort, this study challenges the dictum that most asthma in adults originates in childhood. Studies of the differences between pediatric-and adult-onset asthma may provide greater insight into the phenotypic heterogeneity of asthma.

StreszczenieCzwartorzędowe związki amoniowe (quaternary ammonium compounds -QACs) należą do organicznych związków jonowych o unikalnych właściwościach surfaktantów i środków odkażających. Coraz szersze zastosowanie QACs w środowisku pracy, a także poza nim, niesie ze sobą nowe zagrożenia zdrowotne dla osób eksponowanych i służby medycyny pracy. W artykule przedstawiono wyniki przeglądu piśmiennictwa na temat wpływu amin czwartorzędowych na organizm człowieka. QACs mogą działać drażniąco na skórę i błony śluzowe, a także mieć potencjał uczulający. Podejrzewa się, że poszczególne QACs mogą wykazywać krzyżową reaktywność immunologiczną między sobą oraz z innymi związkami chemicznymi o podobnej budowie, np. z lekami anestetycznymi. QACs mogą promować rozwój alergii w obrębie dróg oddechowych, jakkolwiek mechanizmy leżące u podstawy tego działania wymagają dalszych badań. Dotychczas opisano kilka przypadków astmy zawodowej wywołanej ekspozycją na QACs oraz udokumentowano ich udział w rozwoju kontaktowego zapalenia skóry. Nie wyklucza się także możliwości wywołania wstrząsu anafilaktycznego przez QACs. Med. Their wide distribution in the work environment and also in private households brings about new occupational hazards. This paper reviews reports about the health effects of QACs. QACs could play a role of sensitizers and irritants to the skin and mucous membranes. It is suspected that particular QACs can display an immunologic crossreactivity between each other and with other chemical compounds containing ammonium ion, such as muscle relaxants widely used in anesthesia. They may promote the development of airway allergy, however, the background mechanisms are still unclear and need to be further investigated. Until now, a few cases of occupational asthma induced by QACs have been described and their involvement in contact dermatitis has been documented. The possibility of anaphylaxis due to QACs cannot be excluded as well. Med Pr 2014;65(5):675-682

Introduction. Occupational asthma (OA) is the most common occupational lung disease in industrialized countries; however, in Western Europe, a decline in the prevalence of OA and changes in etiological factors has been recorded. Data from Central Europe, where in the past healthcare and economy sectors were different, are rare. Objective. The aim of this study was to discover the changes related to OA during a four decade long period in the Slovak Republic. Materials and method. The study included patients whom OA was reported during 1980-2016. All cases of OA were divided into four decades. The prevalence of OA, changes in etiological factors and key diagnostic methods were evaluated. All decades were analysed separately and compared to one another. Results. During 1980-2016, OA was reported in 155 patients. OA was most often found in the second decade (6.4 cases per year), the lowest number of cases was diagnosed in the third decade (2.1 cases per year). Since 1988, an increasing prevalence of OA was observed with the peak in 1992, followed by the significantly progressive decrease during subsequent years. In the fourth decade, a mild increase in OA causes was recorded again. In the first and second decades, agricultural allergens were the dominant agents, which were replaced by chemical factors in the last decade. The most frequently used key diagnostic methods in 1980-2009 were skin tests, during the fourth decade the importance of this method declined. However, specific BPT a serial BPT were more frequently used in the fourth decade. Conclusion. The number of OA cases induced by agricultural allergens declined, which was connected with a decrease of total OA cases. The current problem is the OA induced by the chemicals, which increased significantly in the last decade, even the possibility of underdiagnosed OA cases still exists.

Abstract: WRA declined in France over the study period. The only significant increase concerned WRA related to exposure to quaternary ammonium compounds. Zero-inflated negative binomial and logistic regression models appear to describe adequately these data.
