Sickle-cell disease (SCD) is characterized by frequent and painful vaso-occlusive crises (VOCs).Various treatments have been evaluated over the years. However, a clear overview is lacking.The objective of this study was to systematically review all pharmacotherapeutical strategies in the prevention of VOCs beyond hydroxyurea. We performed a systematic literature search (MEDLINE, Embase, CENTRAL). Eligible studies were controlled clinical trials evaluating pharmacotherapeutical interventions targeting the reduction of VOCs in patients with SCD.Primary outcomes were the number or duration of SCD-related pain days, VOCs, or hospital admissions for VOCs. Secondary outcomes included time to first VOC or hospital admission for a VOC. A standardized data extraction sheet was used. The methodological quality of studies was assessed using Cochrane's risk-of-bias tool. A total of 36 studies were included in this review, covering 26 different prophylactic interventions. The most promising interventions for reducing the frequency of either VOCs or hospitalizations were the oral antioxidants L-glutamine and v-3 fatty acids and the IV antiadhesive agent crizanlizumab. Twenty-three studies did not show any beneficial effect of the intervention under investigation, and 6 studies were either too small or methodologically inadequate to draw conclusions. Because of the heterogeneity of interventions, no meta-analysis was performed. In conclusion, this review identified 3 promising pharmacotherapeutical strategies in the prevention of VOCs in SCD. Los telómeros son regiones de ADN no codificante que se encuentran en los extremos de los cromosomas, cuya longitud además de indicar la esperanza de vida, indica el estado global de salud. El objetivo de la presente revisión sistemática es recopilar la evidencia existente sobre la relación entre la longitud de los telómeros y la calidad de la dieta para conocer el impacto que algunos nutrientes, alimentos y patrones dietéticos pueden tener sobre la homeostasis telomérica y por lo tanto, sobre la salud en general. Material y métodos: se realizó una revisión bibliográfica en la base de datos PubMed para identificar los artículos publicados (en inglés o español) hasta diciembre de 2016 que cumplían los siguientes criterios: incluían sujetos humanos; eran estudios transversales; estudios de casos y controles; estudios de cohortes prospectivos o estudios de intervención; que evaluaban la relación de los nutrientes, alimentos o patrones dietéticos con la longitud de los telómeros. La estrategia de búsqueda incluía las siguientes palabras clave: nutrients or food OR food groups OR diet OR dietary pattern OR eating pattern OR dietary habits OR diet type AND telomere attrition OR telomere length. En total, se incluyeron 19 estudios transversales, cinco estudios de casos y controles, cinco estudios de cohortes prospectivos y dos estudios de intervención, incluyendo aquellos artículos que se encontraron por listas de referencias de otras publicaciones. Resultados: se encontraron asociaciones positivas entre la longitud de los telómeros y la adherencia a la dieta mediterránea y el consumo de verduras y frutas. Los resultados obtenidos para otros nutrientes, alimentos o patrones dietéticos fueron incoherentes, aunque parece que las carnes procesadas, los cereales, el alcohol y las bebidas endulzadas podrían estar asociados con telómeros más cortos. Conclusiones: la intervención dietética, y en particular la promoción de una dieta de estilo mediterráneo, podría desempeñar un papel en la protección de la integridad telomérica. Key words:Telomere. Zinc is an essential trace element for human and plays an important biological role in antioxidant properties. We have been reported that zinc-enriched S. cerevisiae FF-10 contained 392 mg% in the YM basal and 3,193 mg% in the YM optimal medium. Antioxidative activity of FF-10 was tested in vitro models by DPPH (α,α'-diphenyl-β-picrylhydrazyl) radical scavenging activity and lipid peroxidation using linoleic acid (LA) and rat liver homogenate. DPPH radical scavenging activity was higher in the cell-free extract of FF-10 cultured in the YM optimal medium (YMOM) than that in the YM basal medium (YMBM). The inhibition activity of lipid peroxidation using rat liver homogenate was shown in the following order: BHT ＞ YMOM ＞ YMBM and these values were dose dependently. The lipid peroxidation of the control mixture by ferric thiocyanate and TBA methods using LA was increased rapidly as typical peroxidation curve of LA from one day and the antioxidation activity of the cell free extracts by cultivating FF-10 in the YMOM were higher than that of the YMBM. Result of this study indicate that the cell-free extracts containing a high intercellular zinc of S. cerevisiae FF-10 cultured in YMOM showed strong antioxidation capacities in DPPH radical scavenging activity and lipid peroxidation using LA and rat liver homogenate.
